2014-02-06 ACID 0 500 Gal 50000 Universal Well Services Inc. On 2/6/2014, I witnessed Universal Well Service stimulate the Clinton Sandstone formation for PAC Drilling. The backside annulus was completely open during the stimulation operations with no failures. There were no casing failures. A total of 27 balls were dropped. A total of 128,178 gallons of water was used. All of the stimulation equipment operated smoothly for Universal Well Service.

Poulson Drilling drilled a 11 Hole #2 to 715 RBK (rig floor) and set 8.625 casing to 686 RBK with 2 centralizers. The casing was 23 lbs/ft. The bottom of the Berea Sandstone was logged at 575. Universal established circulation with 2 barrels of fresh water followed by 12 barrels of sweep and 5 barrels of a fresh water spacer. Next, the operator pumped 150 sacks of Unifill Lite as the lead cement then 125 sacks of Type 1 cement as the tail cement. The cement was displaced to 656 with 42 barrels of fresh water. I witnessed 22 barrels of good cement returned to surface. The cement held static at the surface after 15 minutes.
1018230798 2014-01-23 Drill / Deepen / Reopen No Violations I witnessed Universal Well Service cement the conductor casing for PAC Drilling. Poulson Drilling drilled a 14.75 Hole #1 to 85 RBK (rig floor) and set 11.75 casing to 61 RBK. The rig floor was 5 above ground level. Universal established circulation with 3 barrels of fresh water. The operator pumped 20 barrels of gel followed by 5 barrels of a fresh water spacer. Next, Universal pumped 125 sacks of Type 1 cement. The cement was displaced to 46 with 6 barrels of fresh water. I witnessed 7 barrels of good cement returned to surface. The cement held static as surface after 20 minutes.
1084848582 2014-12-08 UrbanPreliminary Restoration No Violations I was on location for a urban preliminary restoration. During my inspection, I found the well site graded. Vegetation was well established. The well was equipped with a plunger lift assembly. The tank battery consisted of a 210 barrel production tank, a 50 barrel brine tank and a separator. A locked, chain link fence surrounded the production equipment. The proper identification signs were posted. The proper urban safety equipment was installed. There was a dike surrounding the production equipment. The lease road was in stable condition with a gate at the entrance. Erosion and drainage controls were in place. All drilling equipment was removed. All debris was removed. The pits were filled and closed. Urban preliminary restoration passed.
1138270427 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1822456645 involving multiple compliance issues regarding urban requirements. The well needs to be fenced and at the time of this inspection, no fencing has been installed around the well. The fencing was not waived during the pre-site inspection. Identification posted at the tank battery, has been updated with the required information per 1501:9-9-05(E)(1)(a-e). There was a locked chain link fence with no barb wire around the tank battery. Install the barb wire at the top of the tank battery fence. The 210 BBL storage tank was more than 3 foot above the top of the fencing. The gate at the lease entrance needs to be closed and locked. Identification still needs installed or posted on the gate. The gate was open at the time of this inspection.

PAC needs to: 1) Post the proper identification sign and lock on the access road gate per 1501:9-9-05(E)(1)(f) and a lock. 2) Install the barb wire at the top of the chain link fence at the tank battery. 3) Install the fencing around the wellhead.

1157791334 2018-05-15 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1822456645. There was an identification posted at the well. There was no fencing around the well; however, the well is located in a clearing surrounded by woodlands. The nearest private residence was approximately 500 feet away. The fencing was not waived during the pre-site inspection. The diked tank battery has two storage tanks and a separator. The pick-up lines are secure with plugs and the handles have been removed from the valves. The 210 BBL storage tank has the required urban safety equipment. A domestic gas supply with an open ball valve comes off of the sales line at the separator. There was an identification sign posted at the tank battery, but was missing the required information per 1501:9-9-05(E)(1)(a-e). There was a locked chain link fence with no barb wire around the well. The 210 BBL storage tank was more than 3 foot above the top of the fencing. The access road was stable, with a gate at the entrance and shared with permit #2-3966. No lock or identification sign was installed or posted on the gate.

PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 4) Post the proper identification sign and lock on the access road gate per 1501:9-9-05(E)(1)(f). 3) Install the barb wire at the top of the chain link fence at the tank battery.

1391176671 2014-01-30 Drill / Deepen / Reopen No Violations I inspection found Universal Well Service had completed cementing the production casing for PAC Drilling. Poulson Drilling drilled a 7.875 Hole #3 to 5,390 and set 4.5 casing to 5,329 with 20 centralizers. Universal pumped 225 sacks of Unitropic cement. The cement was displaced to 5,319 with 85 barrels of water. The plug landed and held static for 5 minutes. Refer to the cement ticket for further information.
