-flushed the well with 11 bbls of water followed by 5 bbls of a fresh water spacer. The operator mixed and pumped 100 bbls of Type 1 cement with 3% CaCl and 0.25 lb/sk flakes. The cement was displaced with 7 bbls of fresh water to a depth of 60 GL. I witnessed 7 good bbls of cement returned to surface. The cement held static at surface after 15 minutes. | |
0693845588 | 2015-02-02 | Drill / Deepen / Reopen | No Violations | I was on location for a status check. During my inspection, I found the pits partially closed. I called and spoke with Jason Caldwell of PAC drilling. I instructed Mr. Caldwell that the pit closures were overdue. Mr. Caldwell stated the weather had caused delays in meeting the pit closure deadline. Also, Mr. Caldwell stated the pits would be completed by 2/6/2015. A follow-up inspection would be conducted on 2/6/2015. |
0883213301 | 2015-02-06 | Drill / Deepen / Reopen | No Violations | I was on location to check the drilling pits. During my inspection, I found KDA in the process of solidifying the pits. Chemtech had delivered 2 truck loads of kiln dust (~ 25 tons per truck). |
0914693771 | 2017-12-18 | UrbanProduction Wells | No Violations | I was on location to assess compliance notice # -1164797427. I found PAC still needs to:
Raise the fencing around the tank battery or use low profile tanks. -up lines or remove the valve handles. Post the proper identification sign on the access road gate. Install a lock on the access road gate. Replace the broke hatch lid on the 210 bbl. storage tank. |
1126220025 | 2017-06-15 | UrbanProduction Wells | No Violations | I was on location to assess compliance notice # --up lines still missing locks and the handles have not been removed. |
1135217865 | 2019-02-27 | UrbanProduction Wells | No Violations | I conducted a compliance notice follow-up inspection for compliance notice 1164797427, regarding multiple compliance issues related to urban area requirements. At the time of this inspection, the pick-up lines are secure with plugs but still need locks or the handles removed. The required urban safety equipment has been installed on the production tank. An identification was posted at the tank battery, but was missing the required information per 1501:9-9-05E(1)(a-e). A lock is needed on the chain link fence surrounds the tank battery and the three strand barb still needs added. The production tank extends more than 3 foot above the fencing. The access road gate was installed but no identification was located at the access gate and a lock needs installed on the gate.
PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f). 3) Install the barb wire at the top of the tank battery fencing and a lock on the door. 4) Post the proper identification and lock on the access road gate per 1501:9-9-05E(1)(f). |
1164797427 | 2016-01-21 | UrbanPreliminary Restoration | Violations Noted | I was on location for a preliminary restoration inspection. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The tank battery has a 210 barrel production tank, a 50 barrel poly brine tank and a mechanical separator. There is a dike around the tank battery. Identification signs are posted at the well and tank battery. The proper safety equipment is installed on the production tank. There are metal posts installed around the tank battery for a chain link fence. The lease road is in stable condition. The site is graded. Vegetation is well established. All drilling equipment has been removed. The production equipment has the proper spacing. I spoke with Jason Caldwell, owner of PAC Drilling. I told Mr. Caldwell a compliance notice will be issue for failure to install the proper fencing around the tank battery with locks. Mr. Caldwell stated the fence will be completed by February 5, 2016. Preliminary restoration not passed. |
1229912949 | 2015-01-27 | Drill / Deepen / Reopen | No Violations | I was on location for a status check. During my inspection, I found a rig crew running 2.375 tubing into the well. KDA was on location building the tank battery. A 210 barrel production tank, a 50 barrel brine tank and a mechanical separator were in place. The production equipment had the correct spacing per 1501:9-9-05(A)(2). Safety equipment was installed on the production tank. |

1948369647 | 2018-05-16 | UrbanProduction Wells | No Violations | I conducted a compliance notice follow-up inspection for compliance notice 1164797427. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. The well had signage; however the signage was painted over and difficult to read. There was no fence installed around the well due to a pre-site waiver. The diked tank battery has two storage tanks and a separator. The pick-up lines are secure with plugs but still need locks. The required urban safety equipment was missing from the production tank. An identification sign was posted at the tank battery, but was missing the required information per 1501:9-9--e). A locked chain link fence surrounds the tank battery and needs three strand barb added. The production tank extends more than 3 foot above the fencing. The access road was stable, with a gate just before the tank battery.
PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9--e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9--e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9- 4) Post the proper identification sign and lock on the access road gate per 1501:9-9- |
