0672499092 2018-02-26 Production Wells No Violations The lease road and location was rocked and afforded adequate access to the well and tank battery. The Bates Unit #1-H was equipped to produce as a pumping well. The was equipped with a Oilwell Model 114 unit. The required I D signage was hand written on the unit and tanks. The was a 55 gallon drum ( marked residual waste ) to contain any fluids captured by the leak containment pumpkin on the stuffing box. The 7 to 4.5 annulus had a manifold that was comprised of a pop off valve and gauge. The pressure gauge read zero psi. The tank battery consisted of three (3) 210 barrel stock tanks and one (1) Gas Production Unit (GPU). Proper spacing was observed between the equipment. An earthen dike surrounded the tank battery. The preliminary restoration was delayed by weather considerations ( per Cary LeShock ). No violations identified during this inspection.
0894431155 2018-09-18 Preliminary Restoration No Violations Status check inspection for Preliminary Restoration. My inspection found the lease road and pad in stable condition. The wellhead is equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. Gauges indicate a pressure of 400psi on the tubing and 250psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Bates Unit 2H (PN#24097) and contains three (3) 210 barrel tanks. There is a heat treating, horizontal separator 50 to the north of the tanks and is surrounded by a secondary containment dike. There is a contract pumper currently on location (Tim) and he is transferring production water to a 500 barrel steel tank. I spoke with the U.S. Energy Field Supervisor (Kerry) about the 500 barrel tank and he said that the tank is not permanent and will be removed when they get ahead of the production water. Valves at the wellhead and on the load lines are equipped with locks. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. No violations. Preliminary Restoration is approved.

1354855835 2018-01-17 Preliminary Restoration No Violations The lease road was graveled and afforded access to the well and tank battery. The well was completed as a electric pumping unit. The wellhead was configured with 7 intermediate casing, 4.5 production casing and 2.375 tubing with rods. The 7 X 4.5 annular space had the required annular pressure relief valve and gauge. The gauge read 0 psi. No leaks were identified at the wellhead. The tank battery consisted of three (3) 210 barrel stock tanks and a GPU separator. Proper spacing was observed between the equipment. An earthen dike surrounds the tank battery. The I. D. information was written on the tanks. I spoke with Cary LeShock ( U S Energy ) and requested that he include the Township, County and Lot number on the tank. The restorative work has been completed, but I could not evaluate the level of compliance due to snow cover.
1499103757 2017-06-30 Casing String Witnessed No Violations I was on location to observe the cementing operation of the seven (7) inch intermediate casing. U S Energy drilled a 8.25 hole from 326 to 3322 on misted air drilling. U S Energy ran 79 joints of seven (7) inch casing to3322. Casing hardware included 17 bow spring centralizers and a guide shoe. Per the Tool Pusher ( Cub ) the rig circulated approximately 250 barrels to condition the hole prior to cementing. Hole capacity was approximately 181 barrels. Petroset Cementing Services was the cementing contractor. I found the mix water to be 70 F and pH was 7. Petroset connected to the casing and mixed and displaced a cement slurry consisting of 550 sacks of Class A cement blended with 3% CaCl. Cement weight was verified , by scale, at 14.9 ppg. The wiper plug was displaced with 134 barrels of fresh water. Cement returns were observed to return to surface at approximately 126 barrels into the displacement. An estimated eight (8) barrels of cement slurry was noted. The plug landed at 900 psi and was bumped to 1400 psi for the green cement test for five (5) minutes. No pressure decrease was observed. U S Energy requested that 500 psi be left on the casing through the wait on cement time. No violations identified during this inspection.
1499106700 2017-06-27 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 surface casing. U S Energy air drilled a 12.25 hole to 331 and lowered 316 of 9.625, 36# casing into the wellbore. Casing hardware included four (4) bow spring centralizers and a float/ guide shoe. Petroset Cementing Services circulated 50 barrels of gel and cottonseed hulls prior to cementing. Petroset mixed and displaced a cement slurry ( cement weight ranged from 14.9 ppg to 15.1 ppg ) back to surface utilizing 150 sacks of Class A cement blended with 3% CaCl. Mix water was 60 F and had a pH of 7. Displacement was achieved with 25 barrels of water. Cement returns were observed at 20 barrels into the displacement. The plug landed at 150 psi and was increased to 680 for five (5) minutes without any pressure decrease. No violations identified during this inspection.

1507227610 2017-10-05 Drill / Deepen / Reopen No Violations I found Reliance has been contracted for well stimulation operations on the day of my inspection. Reliance completed stages 5 and 7 while I was on site. US Energy used 500 gallons of 7.5% acid per stage to open the perforations. Both stages had 50 shots. Reliance used no sand on stage 5 and 42 sacks of sand for stage 6.
1533394345 2017-09-14 Completion Testing No Violations Moore Well Services was on location swabbing on the 4.5 casing. Fluid level was approximately 500 down. All recovered fluid was contained in a 500 barrel tank. The pits had sufficent free board of volume. A walk around inspection did not identify any violations.
1610346920 2017-07-05 Drill / Deepen / Reopen No Violations I was on location to observe the progress of drilling and the status of the gas migration previously witnessed by DOGRM Inspector Skidmore. Navigator Wireline Services was running a Gyro log for orientation purposes. The 9.625 to 7 annular space was not open for inspection. A cement cap had been placed on the top of the 9.625 casing and beveled up to the 7 casing. I spoke with Cub ( tool pusher ) concerning getting the blow line deeper into the pit to minimize any drifting of rock cuttings. He said, they would fix it. I spoke with Cary Leshock ( U S Energy Superintendent ) who confirmed that a seven (7) inch casing head would be utilized on all subsequent wells and the blow line would be constructed in a manner that would eliminate any potential drifting of effluent material from drilling operations. No violations identified during this inspection.

The pits offered sufficient room. The crew was unloading 4.5 production casing. A walk around inspection of the location, back yard, cellar and mud pump did not indicate any violations.
2093344825 2017-11-28 Preliminary Restoration No Violations Access to the Bates Unit pad was gained on a graveled road. The stoned pad encompasses an area approximately 200 X 180. The silt fencing along the lease road and perimeter of the pad was functional and effective. The pits were previously closed, seeded and strawed. The tank battery was constructed on the west side of the pad. Three (3) 210 barrel tanks and one (1) Gas Production Unit (GPU) comprise the production equipment. The spacing requirements were observed in the construction of the tank battery. The well was completed as a electric pumping well and was waiting for electric service. Several non-compliant items were identified during my inspection. I brought to U S Energy personnel ( Cary LeShock, Harry ) attention that the I. D. signage needed to be posted, the pop off valve for the 7 X 4.5 annular space needed to be attached and several areas on the perimeter of the pad required seeding and straw placement for stabilization. Preliminary restoration was not passed.
