0125021547 2017-10-30 Completion Testing No Violations The access was graveled to the well location. Nineteen 500 barrel frac tanks were on location to be used in the completion of the Williams # 1-H well. The pits had adequate room. Moore Well Service ( MWS ) was working on the Williams # 2-H well. Having previously drilled out the frac plugs, MWS was rigging up to begin swabbing operations in the 4.5 casing. No violations were identified during this inspection.
0216053480 2017-09-29 Drill / Deepen / Reopen No Violations The lease road and location were graveled and accessible. U S Energy Rig # 9 was drilling on a salt water based fluid at 4764 MD. Rate of penetration was 35 / hour. Background gas was measured at 8 unit units. Initial gas show were reported after the top of the Lockport Dolomite was penetrated at 3312 MD. The circulating pits had adequate free board. The cellar was free of any gas migration. The backyard ( air packs, mud pumps ) were inspected and found free of leaks. No violations identified during this inspection.
0295368830 2017-11-01 Completion Testing No Violations Moore Well Services was swabbing the 4.5 casing from approximately 2600 deep. A 500 barrel Frac tank was used to contain the swabbed fluids. The well had 35 psi prior to be opened. The pits will remain open until the Williams # 1H ( 34-155-2-4101 ) is completed ( scheduled for 11/13/2017 ). The pits had adequate free board. No violation identified during this inspection.
0323897404 2017-09-26 Drill / Deepen / Reopen No Violations U S Energy was drilling on misted air at 3614 MD. The kick off point was 3380 TVD in the Lockport Dolomite. The lease road and location were graveled and accessible. The pits had adequate room and freeboard. The cellar was inspected and found free of gas migration. The air compressors and booster area was free of leaks. No violations found during this inspection.
0567806856 2018-08-07 Preliminary Restoration No Violations I was on location to check the status of Preliminary Restoration of this horizontal, Clinton Sandstone well. My inspection found the lease road and well pad in stable condition. The well is in production and equipped with 2.375 tubing and rods and operating with the aid of an electric powered pumping unit. Gauges indicate pressures of 300psi on the tubing and 450psi on the 4.5 production casing. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. The tank battery is diked and is shared with the Williams Unit 1H (PN#24101). The battery contains three (3) 210 barrel tanks and the separator is located in a separate containment area east of the tanks. All of the load lines are locked. There is an electric pump in the northwest corner of the battery that is plumbed to the load lines, the top of the western tank, and to the top of a 500 barrel frac tank that is sitting outside of containment. The fluid level in the frac tank is 4. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. I will speak with U.S. Energy about the status of the 500 barrel tank prior to approving Preliminary Restoration. If the tank will be permanent, secondary containment measures will need to be installed around the tank. No violations.
1005306417 2017-11-28 Preliminary Restoration No Violations The pad was accessed on a gravel road. The tank battery was not constructed at the time of my inspection. The Williams # 2-H well was equipped to operate as a pumping well. The pumping unit was not set yet. The pits remained open and have adequate volume. The remains assorted equipment on location, i.e. Frac tanks ( 16 ), air compressor, piping, etc. I will contact U S Energy to confirm a closing date for the pits and removal of the equipment which was not required for production activities. Preliminary restoration was not passed.

1409624478 2017-10-26 Drill / Deepen / Reopen No Violations I was on location to witness Producers Service Corp. (PSC) stimulate the well for US. Energy during stage 6. Surefire is the wireline company on location. US Energy shot 50 perforations for the stage. PSC used 15% acid to open the perforations. PSC used 220 sacks of sand. The port on the 8.625 casing is open.
1504786644 2017-09-07 Drill / Deepen / Reopen No Violations I found US Energy not working on the day of my inspection. The site is secure with a forklift blocking the entrance to the well pad. Rig 9 has been skidded into position over the cellar for the well. The pits are lined and in good repair
1506093413 2017-09-22 Drill / Deepen / Reopen No Violations U S Energy Rig 9 had connected the annular Blow Out Preventer (BOP) to the 9.625 intermediate casing and closed the annular BOP on the 3.5 drill pipe. The annular BOP closed with 1300 psi at the accumulator. The pressure of the 9.625 casing was increased, using the soap pump, to 250 psi and I requested that it be released in order to make sure that the system was not boxed in. After closing the annular BOP the 9.625 pressure was raised to 780 psi and held for 15 minutes. After 15 minutes the pressure declined to 740 psi ( 5% decline ) at the end of 15 minutes. The BOP test was passed. The pits had adequate room for liquids. I did not find any gas migration in the 9.625 X 13.375 annular space. A walk around tour did not indicate any violations during this inspection.

1507121077 2017-10-04 Drill / Deepen / Reopen No Violations I found US Energy moving drilling equipment off location on the day of my inspection. Rig 9 has been removed. There is an orange safe fence surrounding the drilling pits. The well is currently shut-in. An identification sign is posted.
1507228814 2017-10-05 Drill / Deepen / Reopen No Violations I found US Energy removing all of the drilling equipment from the well pad. I contacted Robert Letavish from complaint #5687. I informed him that drilling of the well is completed. Mr. Letavish knowledge and was glad the night time operations were done.
1521038015 2018-02-26 Production Wells No Violations The lease road and location was rocked and offered adequate access to the well and tank battery. The Williams # 2-H was equipped to operated as an electric pumping well. The well was equipped with a Oilwell Model 80 pumping unit. Electric power was supplied by a generator until an electric connection is made. The I D information was written on the pumping unit and stock tanks. There was a 55 gallon drum ( marked residual waste ) connected to the leak containment pumpkin on the stuffing box. The 7 to 4.5 annulus had a manifold that included a pop off valve and gauge. The gauge read zero. The tank battery consisted of three (3) 210 barrel stock tanks, and one (1) Gas Production Unit (GPU). An earthen dike surrounded the tank battery. The erosion controls, on the south side , were effective. The preliminary restoration was not completed at the time of inspection. No violations identified during this inspection.
1607142743 2018-01-17 Preliminary Restoration No Violations The lease road was graveled and allowed access to the well pad. The drilling pits were closed. The top soil remained in a pile on the east side of the pad. Weather permitting the top side will be spread as soon as possible according to Cary LeShock ( U S Energy ). The well was equipped to operate as a pumping well. Visible well construction included 7 intermediate casing, 4.5 production casing and 2.375 tubing with rods. The annular space between the 7 casing and the 4.5 casing was open and free of any gas migration indications. Mr. LeShock said, that the pressure relief valve and gauge will be attached when the pumping units are set. All the valves on the wellhead were locked. Three (3) 210 barrel stock tanks were on location, but the tank battery construction had not begun. No violations were identified during this inspection.
1983590554 2018-09-18 Preliminary Restoration No Violations Status check inspection for Preliminary Restoration. My inspection found the lease road and pad in stable condition. The wellhead is equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. Gauges indicate a pressure of 250psi on the tubing and 420psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Williams Unit 1H (PN#24098) and contains three (3) 210 barrel tanks. There is a heat treating, horizontal separator 50 to the east of the tanks and is surrounded by a secondary containment dike. There is a contract pumper currently on location (Tim) and he is transferring production water to a 500 barrel steel tank. I spoke with the U.S. Energy Field Supervisor (Kerry) about the 500 barrel tank and he said that the tank is not permanent and will be removed when they get ahead of the production water. Valves at the wellhead and on the load lines are equipped with locks. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. No violations. Preliminary Restoration is approved.
