0975326535 2014-03-07 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well was capable of production equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing and a plunger lift system logically controlled. Production equipment included a 100bbl tank, a vertical separator, a 2 inch meter with a 1.25 inch orifice and a chart recorder. All valves were open at the well head, flow line and sales line. Identification was present and production records up to date. Site passed preliminary restoration according to 1509.072A.
1062957070 2018-07-24 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped 4.5 inch diameter casing, 1.5 inch diameter tubing, and a plunger lift system. The valves were open on the casing and on the tubing. Production equipment included a steel 140 barrel tank, a vertical separator, a plastic 50 barrel tank, and chart recorder with a 2 by 1.250 inch orifice plate within an earthen dike. The production report was up to date and proper identification was present.
1258164486 2006-11-22 Drill / Deepen / Reopen No Violations Check this well because of a fire and I was the closest Inspector near Drilling rig .I found that fire was out , rig was at 883 ft. when a blowout occurred and the fire began .The fire was set with the stones coming out of the hole hitting the sub-base and the derrick which caused a spark .The fire burnt half way up the derrick and was out in about 1 hour .During that time the wheels on the rig ,the control box and most of the hydraulic controls and valves melted Also the Kelly hose was burn in two .Water trucks need to haul fluid away and another pit needs to be constructed before Hallibuton can circulate gel and kill well .There is no pit space left for this .The pits have run over caused by the blow out .Killbarger is the drilling contractor .Halliburton is on site and the pits are being constructed so that Halliburton can circulate and kill well .Contact Inspector Grimm and advise of my findings along with Supervisor Cheslock .
1998221817 2016-04-21 Production Wells No Violations On the day of my inspection I found an 8 5/8 X 4 1/2 wellhead and a 4 1/2 X 1 1/2 inch wellhead operated by a plunger lift system. The valves are in the open position. The tank battery is made up of one 100 bbl. tank, one 30bbl. Tank, a vertical separator, and a recording gauge, all in an earthen dike. The tank is properly identified, the production reporting is up to date and there are no violations at this time.
