0376141819 2017-12-12 Final Restoration No Violations On the day of my inspection I went to the provided well coordinates. I was unable to find a well or production equipment. Area is well vegetated with no signs of leaking well casing. Pipeline riser remains onsite. I spoke to Joe Snyder of Artex on 12/20/2017, who stated that the pipeline is still is use for other wells and is under a pipeline right-of-way agreement with landowner. Site meets requirements of ORC 1509.072B for final restoration.

1470242680 2016-08-03 Plug / Plug Back No Violations Upon arrival on location I found the rig preparing to plug and abandon the well. The rods have been removed and they are dismantling the wellhead and getting ready to remove the 2 3/8 inch tubing from the wellbore.

A 30 sack Class A plug was set. The 2 3/8 inch tubing will be pulled out of the wellbore and the well will be shut-in for the weekend.

1932393472 2015-04-30 Production Wells No Violations On the day of my inspection I found a 8 X 4 wellhead and a flanged 2 inch wellhead with tubing and rods, operated by a gas powered motor on a pumping unit. The valves are in the open position. The tank battery is made up of one 210 bbl tank and a horizontal separator, all in an earthen dike. The tank is properly identified and the production reporting is up to date. There are no violations at this time.
