0042951421 2012-12-06 Orphan No Violations As preparing estimate for plugging, realized that still needed a ROE to access well from neighbors property, which was the original plan. When I met with landowner, we found a better way, crossing the creek, that would not require other landowners. This way will probably be less expensive also.
0383969723 2013-04-02 Orphan No Violations On a routine visit with Ohio Soil & Water inspector Bob Mulligan, found that this well had recently discharged some oil and oil was now laying around the casing in an area about 2 x 3. Since this well lays in a dry part of the creek bed, well should now be referred to the Coast Guard for assistance in plugging. We are not certain what caused the discharge of oil, possibly heavy rains causing the creek to rise and flood over the well. Will continue to monitor.

0885446028 2014-08-11 Plug / Plug Back No Violations 7:00 AM - MH crew arrives and begins clearing brush around the house to access creek crossing and well, 8:30 AM - I arrived on loation. Porta-John delivered. Two loads of rock brought for access road between 1st creek crossing and the township road. Trackhoe spends the morning spreading rock, pulling out trees and working on access to the well. 12:00 PM - Both big trees are down, cut up and chipped. Trucks with timbers arrive and are unloaded. Timbers used at 2nd creek crossing and around the well to form a solid foundation for the rig. Trackhoe continues to clear brush around the well. 5:30 PM - Rig moves on location and I leave location., 6:30 PM - Crew leaves location.

1013354578 2014-08-13 Plug / Plug Back No Violations 7:30 AM - MH crew arrives, 9:45 AM - I arrived on location. The crew had prepared the access and location with trackhoe for the cementing equipment, loaded the water tank with fresh water, and moved the old tubing off location. 10:00 AM - The new tubing arrives and is hauled to well by the trackhoe. 11:00 AM - The crew runs the 2 3/8 tubing into the well to cement through, tagging bottom at 1049 and pulling back up to 1020. 2:15 PM - Huffman Bowers Cementing (HBC) arrives and the bottom plug is cemented from 1020-620, using 70 sacks of Class A cement weighing 15.5#/gallon. The well circulates @ 15 barrels of water-cut oil into the steel half tank. 2:45 PM - The crew removes the tubing from the well, 3:45 PM - HBC cementers and I leave location, leaving cement trucks on location for tomorrows cementing. 4:30 PM - MH crew leaves location.
1104082327 2014-08-12 Plug / Plug Back No Violations 7:30 AM - MH crew arrives on location, 7:45 AM - I arrived on location. Trackhoe unloads steel half tank and places it near well. 9:15 AM - Crew pulls rods slowly to prevent well from unloading, and recovers 1020.7, with travelling valve - standing valve still left in hole and it needs to be removed so tubing can be pulled without mess. Crew decides to go back in hole with fishing tool on rods to recover standing valve but are not sucessful. Crew removes old wellhead and welds a working one onto the 6 5/8 casing, to control any flows or gas from the hole. 2:15 PM - The crew begins to pull 2 3/8 diameter tubing, using a fishing tool (spear) to access the rotten pipe and collars, as needed. Crew recovered 1044.7 of tubing, much of which has large holes and all will be salvaged. The crew will bring additional 2 3/8 tubing in the morning, to cement through. 7:30 PM - I left location, 8:00 PM - MH crew leaves location.
1334326026 2012-04-12 Orphan No Violations My inspection this day for preparation of orphan well referral. I found well that had been in middle of creek 2 years ago now located on creek bank in a thick layer of silt and brush debris that has come down the creek. The well has rods and 2 3/8 tubing with the top of the rod about 10-12 above ground level. There is a square of concrete around the 6 casing and no sign of conductor or surface casing. The wellhead is shut in and oil has run out from one of the plugs. When I first visited well, there was gas bubbling thru the water out and around the outside of the casing. No sign of leakage now other than the oil staining that ran down the side of the casing. I have picked this permit number to call this well, even though it is not plotting very close to the location on the map. The old log shows well was drilled to 1049 and drilled in 1941. Easy access off Cty Rd. 48, about 3 miles north of St. Rt. 60.

1426018228 2014-10-16 Final Restoration No Violations My inspection found that all the production and plugging equipment had been removed and the creek restored to its original, if not better condition. What remains of the trees that were not chipped up will be used by the landowner for firewood. The ground disturbed had been smoothed, seeded and mulched and the grass was beginning to grow.
1440620417 2014-08-14 Plug / Plug Back No Violations 6:45 AM - MH crew arrives, 7:30 AM - Appalachian Well Surveys (AWS) arrives on location, 8:15 AM - I arrive on location and AWS verified the top of the bottom cement plug to be at 703 and that the bottom of the 6 5/8 casing is at 595. The bond log indicates that there are tight spots and that the 6 5/8 casing cannot be pulled so the casing is perforated at 116-121 and 438-443 to allow the cement to get behind the casing. The 6 5/8 diameter casing is filled to surface with fresh water before the bond log was run but water level decreased, indicating there may be holes in the casing. 11:30 AM - Tubing is run into the 6 5/8 casing to 690 and well is ready to cement to surface. 1:00 PM - Huffman Bowers Cementing crew arrives. The well is still showing a little gas and oil as it circulates. The well is cemented from 690 to surface with 185 sacks of Class A cement weighing 15.5#/gallon. Cement circulates to surface. The fluids in half tank are removed and the oil is taken to a separate tank to settle out and the brine/fresh water mix are taken to a disposal well. 2:05 PM - Tubing is removed from the hole and the crews clean up. Cement is found about 50 down from surface and will be topped off in morning 3:45 PM - I leave location, 5:00 PM - MH crew leaves location..
1776320444 2013-07-11 Orphan No Violations My inspection this day due to flooding rains last evening and I wanted to see if this well had released oil also. I found much debris piled up around the well and water had been over the wellhead, yet I didnt see any oil in the creek or around the well. This well has been accepted into the Coast Guard program to plug. One picture taken.
1831443416 2014-08-06 Orphan No Violations I talked with Mark Hines, plugging contractor, and he said he would start delivering his equipment in on 8/8 and should be ready to commence plugging operations on 8/11/14. The deadline of his contract is 8/15 and I requested that he file a 30-day extension, to which he agreed. This day I also called and notified Charles Poole, the adjoining property owner to the north, of the timeline of activities on the Heldman property. He requested that we did not go on his property or turn around in his driveway, to which I agreed.
1843540608 2014-08-15 Plug / Plug Back No Violations 6:45 AM - MH crew arrives and begins tear-down and reclamation. 9:30 AM - I arrive on location. Trackhoe removes timbers used as the well pad and creek crossing and they are loaded on trucks. The top of cement in the hole is found to be @ 50 down and after the casing is cut off, 10 sacks of dry cement (and water) are added to bring the cement to surface. The steel plate with permit number is tack-welded to the casing and the hole is covered over. MH crew finishes reclamation. 8:00 PM - MH crew leaves location.
