0826086287 2016-03-21 Production Wells No Violations Upon inspection, it was observed that the contaminated soil has been removed from inside and outside of the containment dike, the dike walls have been rebuilt, and the soil has been limed, seeded, and mulched. This satisfies the requirements of the compliance notice dated 3/15/2016.
1639906425 2016-03-14 Production Wells Violations Noted Received call from SE Region Supervisor Brent Bear about an anonymous complaint of an overflowing production tank associated with this well. Upon inspection, it was observed that the 100 bbl tank had overflowed and released oil and brine into the containment dike surrounding the tank. The dike wall is breached in the SE corner and allowed brine and oil to flow approximately 60 downhill from the tank battery. The operator knows of the contamination and has initiated cleanup efforts by spreading peat moss over the oil soaked areas to help slow its migration. It is unclear how much product was lost from the tank, however there is <0.5 bbls of oil and brine laying inside the containment dike area. There is 1 of headspace in the top of the tank, indicating some product was removed after it overflowed. Field testing with Hach Quantab Titration Strips showed a chloride level of over 6215 ppm in the fluid laying inside the containment dike. Photos were taken to support the findings of this inspeciton. The operator, David Pottmeyer, shall meet the following requirements within 14 days of receiving this notice: 1.) Remove all contaminated soil from inside and outside of the containment dike and dispose of it at an OEPA approved landfill; 2.) Reconstruct the containment dike walls to meet SPCC guidelines; 3.) Lime/fertilize and seed/mulch the affected area to encourage vegetation growth.
2131345672 2004-05-24 Production Wells No Violations Inspection this day found well producing, current chart in meter, big 200-bbl tank with no dike, good IDs but wrong permit no. - I corrected it.
