0834498378	2015-09-22	Production Wells	No Violations	Upon inspection, it was observed that this well is currently in production. The visible well consists of 4.5 casing with a 4-2 swedge, 2 ball valve, and associated 2 flow lines. There is still no ID information posted at the well or 100 bbl tank. I will contact Mr. Bill Stevens and instruct him to have ID posted by the end of next week.
1215057980	2014-06-23	Production Wells	Violations Noted	This well was inspected in response to a complaint. Upon inspection, it was observed that the well is capable of production. The well consists of a 4.5 casing with a swedge to a 2.5 gate valve, and is equipped as a gas well only. The 2.5 gate valve was in the closed position. The gate valve on the line coming from the 4.5 casing was in the open position, and is running to a Dominion gas meter at the property entrance. The valve is currently off at the meter due to Dominions compressor not functioning. The tank battery for the well consists of a 100 bbl. production tank and a drip sitting inside a containment dike. There is no identification at the well head or the tank battery. Mr. Bill Stevens was notified of this situation in person, and has verbally agreed to put new I.D. tags on this well as soon as possible.
