0763403896 2014-11-26 Plug / Plug Back No Violations On location to witness plugging activities for this well. Upon arrival, Appalachian Well Surveys (AWS) was on location preparing to tag the top of the last plug and shoot the 4.5 casing off to allow for a surface plug to be set. AWS ran their tools in the hole and sat down at 265, indicating that the perforations at 1100 may not have taken any cement. Vircos crew ran their jar and stem into the hole and also sat down at 265, they jarred up and down aggressively to rule out any obstructions other than cement. The bottom was very solid and the tools came out clean, which indicated a cement bottom. It was then permitted for AWS to shoot the remaining 4.5 casing off at 255. After shooting the casing off, Virco used their rig to pull on the casing and made sure it was free. Formation Cementing then established circulation and pumped a surface plug from 252-0 using 130 sks 2% gel cement. The remaining 4.5 casing was then pulled from the hole and the cement was topped off to the surface and it did not drop.

The pits are filled, the location is back to grade, production equipment has been removed, the lease road is restored, and there is a full vegetative cover established. 1416608211 2014-11-21 Plug / Plug Back No Violations On location to witness plugging activities for this well. Upon arrival, Virco Inc. had run tagged TD at 3049, which confirmed the well had been plugged back to produce the Gordon Sand. 91 joints of 1.5 tubing were ran into the hole to 3020. There were 24 open perforations from 2958-2697. Formation Cementing pumped a 659 bottom plug using 50 sks class A cement, 25 lbs cellophane flake as LCM, and 6.2 bbls of freshwater displacement. The bottom plug will be left to set up over the weekend and Appalachian Well Surveys will be brought on location Monday to tag the plug, find free casing, and shoot the 4.5 casing off.
1416874755 2014-11-24 Plug / Plug Back No Violations On location to witness plugging activities for this well. Upon arrival, Appalachian Well Surveys (AWS) was setting up to tag the top of the bottom plug and run a bond log on the 4.5 casing. The top of the bottom plug was tagged at 2402. The bond log results yielded significant evidence that the annular area of the 4.5 casing had caved in multiple areas throughout the well bore. In light of this evidence, it was permitted for plugs to be set through perforations shot at target zones. AWS shot 4 holes in the 4.5 casing at 2370-2371. Vircos crew then ran 1.5 tubing into the hole to 2382 and Formation Cementing pumped a 150 plug from 2382-2232 using 25 sks 2% gel cement. Virco then pulled 20 joints of 1.5 tubing and AWS shot 6 holes at 1780-1781. Virco then ran 2 joints of tubing back into the hole and Formation pumped a 200 plug from 1785-1585 using 48 sks 2% gel cement. Virco then pulled tubing to 1055 and AWS shot 6 holes at 1100-1101. Virco then ran 2 joints of tubing back into the hole to 1122 . Formation then pumped a 150 plug from 1122-972 using 57 sks 2% gel cement. Good circulation was established on all plugs and there was approximately .5 bbl flowback at the pump truck for the last 2 plugs. This indicates that cement had made it through the perforations and packed off the annulus of the 4.5 casing. AWS will return on 11/24/2014 to tag the top of the highest plug and shoot the 4.5 casing off. Formation will also return on 11/24/2014 to pump the surface plug.
