1027635943 2015-10-01 Drill / Deepen / Reopen No Violations Upon inspection, it was observed that this well is still flowing after its frac job. The frac valve is still on the 4.5 casing and there are 7 210 bbl frac tanks on location. There is a flow line leading to a separator, drip, and meter run which are set temporarily until the well is sold dead. I spoke with James Williams in person and he stated that tubing and rods will be run in and a pumping unit will be set once the well dies down. The area downslope of the location has been stabilized with a vegetative cover, the area around the well head is still barren, but is compacted well and is not eroding. Mr. Williams stated they will have the entire preliminary restoration completed after the pumping unit and tank battery is set. I was accompanied on this inspection by Inspector Brian Sefcik.
1411758512 2014-09-24 Drill / Deepen / Reopen No Violations On location to witness cementing of 100 of 9.625 conductor casing. Monroe Drilling is the drilling contractor for this well. The depth of the hole was 110 (100 of conductor, 10 of rat hole). Formation cementing pumped 9.2 bbls of freshwater ahead to establish circulation, then circulated 100 lbs of gel to condition the hole. 50 sks of class A cement were then circulated into the well with LCM and good cement returns were seen at the surface. The cement dropped approximately 18 below surface and stayed. The tail cement was run with 150 lbs of calcium chloride to accelerate setup time. A cement ticket was received after the job was completed.
1418345487 2014-09-25 Administrative Inspection No Violations Spoke with ODNR DOGRM Permitting Manager Steve Opritza at 12:19 pm via phone call to discuss how much surface casing could legally be run and cemented in this well. Mr. Opritza stated that since the USDW was undefined in that part of Washington County and there has been 100 of conductor casing cemented to surface, the surface casing could be set through the Big Injun to hold back fluid and make drilling easier. I relayed this information to Mr. Jim Williams of Gas Enterprise Co. and to Mr. Kerry Brown of Monroe Drilling.
1539254281 2014-10-01 Drill / Deepen / Reopen No Violations On location at 9:00 am to witness surface casing pressure test taking place. Upon arrival, Monroe Drilling rig hands were rigging up pressure testing equipment to the well head. At 10:00 am, the well was pressured up to 600 psi and was monitored for 30 minutes. The well satisfied the pressure test requirements.
1712081010 2014-09-30 Drill / Deepen / Reopen No Violations On location at 3:00 pm to witness surface casing cement job. Upon arrival, Monroe Drilling was still running 7 surface casing into hole. Casing was landed at 1510. TD was 1560, leaving 50 of rat hole below the shoe. Pumping commenced at 7:27 pm and circulation was established at 7:48 pm. Good cement returns were seen at the surface at 8:20 pm and the wiper plug down time was 8:25 pm. Cement did not drop from the surface after monitoring for 30 minutes. Cement ticket was received after the surface job was finished.
1803756190 2014-09-29 Drill / Deepen / Reopen No Violations Visited this location to check the status of drilling operations. The hole was at 360 and Monroe Drillings air rotary rig was drilling with an 8.75 bit. Mr. Kerry Brown of Monroe Drilling stated that TD for the surface hole should be reached by early morning on 9/30/14 and the cement job should commence at around 12:00 pm that day.
1889453117 2016-06-06 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing and 4.5 inch production casing. The 4.5 inch casing is equipped with a casing valve, a 4.5 inch to 2 inch swage nipple, and a 2 inch ball valve. Valves are opened and pressure on the gauge is reading 70 psi. Identification is located at the well head. The production equipment consists of one vertical separator and seven 210 bbl. tanks with only one tank in use. This well is producing into the same tank as #9751. The site has been restored with no erosion. Location has met 1509.072 A requirements.
