0438810106 2018-08-10 Final Restoration No Violations I am accompanied by inspector Tom Thomas for training purposes. Our inspection finds that the road and the location have been reclaimed. A few small pieces of pipe are located at the west edge of location along with a steel drum and some electric wire. I called Bob Handschumaker production manager for Triad Hunter LLC. and informed him of my findings. Bob stated that he would have the items removed by the end of next week.
0645272697 2018-02-08 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. Patchwork Oil and Gas had their rig at the beginning of the lease road and plans on plugging this well and 34167611290000 when the ground is not so wet. I spoke with Bob Handshoemaker the superintendant of Blue Ridge Mountain Resources about this well and he informed me that they are planning to plug the wells very soon and that is why the service rig is there.

1172684857 2018-02-28 Plug / Plug Back Violations Noted I received a call from Roger Whited owner of Patchwork Oil of West Virginia at 7:47 AM on 2/28/18 and he told me that this well had begun to flow on Tom Billy and his rig crew the day before. When I arrived on site there were absorbent pads on the ground and there was about less than a barrel of oil on the ground. Most of the oil had been cleaned up the previous day and there were mainly just a few spots of still standing oil. The area where the oil had gone was about an 8 feet long by 10 feet wide area. Patchwork had dug a small ditch to catch any oil and build several dams around the area to keep the oil contained. Patchwork had cleaned up most of the spill the day before but there was still some oil on the ground. Patch work arrived on location around 9:30 AM and I informed them they would need to get as much of the spill cleaned up as to not let it run down the hill. Patchwork will continue to monitor the oil as we continue to work on the well for plugging purposes and will clean the spill as they restore the well site once the well is plugged.
1488087295 2009-05-05 Production Wells No Violations Inspection this day found well producing with jack. Did not walk down to it but heard it pumping after pumper started it - location taken from circle on topo map. Kevin L., the pumper says these 3 wells in line and are all Berea, going down to ship tanks below the wells along old train tracks. Triad is using wrong permit numbers and turning production in on wrong wells. This may be what they are calling 60490 or 60411.
