Zero (0) production was reported for 2017, but no production was reported prior to 2017. Updated pictures were taken. |

There is no tank battery or lease road associated with this well. There is no identification at the well head. A well head exists at the surface with a metal plate on top with two holes cut in it. I shoved a stick in it and could only get it 6 feet down. There is also tubing in the well underneath the metal plate. I have not found any plugging records for this well. There are also no production records for this well. On the site there are miscellaneous pieces of steel pipe and fittings. Pictures were taken during this inspection. Identification needs placed at the well. Production records need submitted. Well needs to be made capable of commercial production within 60 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond. |

Upon inspection, it was observed that the well is idle and incapable of production. The well consists of a 10 casing with an open well head that once housed 2 tubing and 1 tubing for a gas escape packer setup. The 1 tubing is still in the well. Previous inspection reports indicate that the well may have been plugged at some point, this is not the case, as I was able to push a 10 section of 1 tubing down into the well and heard debris fall into fluid in the well approximately 5 seconds down hole. I spoke with I&O Inspector Cindy Vandyke and she said that this well likely belongs to Mr. Carl Heinrich, as he purchased all of US Oil & Gas wells several years ago. I will check court house records and contact Mr. James Vuksik of US Oil & Gas as well as Mr. Carl Heinrich to inquire about the ownership situation for this well. |

The well owner/operator will be notified if the plugging schedule is approved or denied. |
