-7:45 am: Left Zanesville and mobilized to location - -10:30 am: Water level in well bore began to rise, indicating that the thief zone is beginning to seal off. -10:50 am: Crew left location to retrieve more clay. -11:00 am: Left location. 1:00 pm: Back on location, crew just arriving with another 2 ton load of clay. Crew decided to take a break to check the depth of P#66540. 2:00 pm: The crew began washing more clay into the hole and clay began to show up in the creek. This indicated that clay is being forced into the thief zone and should plug off soon. 2:20 pm: The creek began to clear up and the water level began to rise again in the well bore. 3:15 pm: Left location as Bethel O&G crew cleaned up for the day. Clay will be allowed to settle for a few days to see if water level drops again. |

-3:48 pm: On location to witness grouting of surface plug on this well. Bethel O&G crew and Smith Concrete Redi- -3:50 pm: Crew began filling hole with grout using 5-gallon buckets. -3:55 pm: Grout had been coming up-hole, but began to drop under its own weight. This indicates that the grout is forcing its way into the thief zone. The decision was made to allow the grout to set up for 30-60 minutes to gain some load bearing capacity before attempting to grout the rest of the way to surface. -4:40 pm: Began grouting again, and the grout level began to come up-hole again. -4:45 pm: Grout at the surface. This plug took approximately .25 yards of grout to fill to surface. - -5:00 pm: Annulus is grouted to surface. Smith Concrete began washing down the Redi-Mix truck. The truck operator stated that a total of 1 yard of grout was used between both wells. -5:15 pm: Left location as crew Smith Concrete left and crew cleaned up for the day. The crew will spend the next few days rigging down, grading the location, and preparing to cut the casings below grade to weld the identification plates on. |
0559898950 | 2015-01-23 | Final Restoration | No Violations | My inspection this day to confirm that reclamation has been completed for this well. The final grade is satisfactory and the rocks from the pad constructed for plugging have been spread along the creek bank to prevent erosion. The location has been seeded and mulched but no grass is growing yet. I will check back in several weeks. Final Restoration not passed. |

-7:45 am: Left Zanesville and mobilized to location -9:05 am: Arrived on location, Bethel O&G crew on location at 9:10 am. -9:30 am: Rigged up different style of bit (star type) in an attempt to speed up drilling. -10:00 am: Could not get star bit down hole. Bob Lane will attempt to grind down the sides of the bit to prevent it from catching on the casing/well bore. -11:00 am: Star bit still will not go down hole and is catching the casing and lifting it up on its way out of the hole, further indicating that not very much casing is in the well bore. - -11:45 am: Back on bottom with original bit, drilling. - |

1272881037 | 2010-05-01 | Orphan | No Violations | Call from OEPA regarding well leaking into PawPaw Creek. Joyce Gill called and her husband met me - well casing about 5 below ground level putting sheen on water. Top of casing about 1 above water level, 6.62 casing. Believe it is old well that has been plugged, creek has eroded bank and washed it out into creek. Went to look at this evening as flooding expected for tomorrow and would not be visible. Pictures, will begin I/O application. |
1333999385 | 2012-04-09 | Orphan | No Violations | My inspection this day found well top is again above the surface of the water. I can see oil laying inside the casing and there are ribbons of oil on the water surrounding the well. |

1418016269 | 2010-06-08 | Orphan | No Violations | Inspection this day found water down below top of casing this day, slight oil film on water around well. Big rains this weekend, previous 2 or 3 times I have checked it did not see any oil film. Debatable whether well should be opened and re-plugged. |
1431021080 | 2015-05-07 | Final Restoration | No Violations | Today, the Bob Lane Welding crew added several loads of large stone to the bank, upstream of the well location. This area was starting to erode and threatened the integrity of the well location. This area was topped with several loads of soil and seeded. The grass is growing nicely and this work finishes up the reclamation work. Final Reclamation passed. |

-7:30 am: Started day, had to stop into Zanesville office to switch out vehicles. -11:25 am: Arrived on location. Bethel O&G crew still drilling on cement plug. -11:45 am: More oil showing up in Pawpaw Creek than previous days. Crew placed oil absorbent pads on top of the water to assist the boom in absorbing any free oil. - -1:30 pm: Drilling pace slowing down once again, the cause seems to be the cuttings settling out quickly in the hole and clogging up the contact area between the bit and the cement. - |

Tools were run into the hole to check how much fill- -3:30 pm: Approximately 2.5 tons of clay has been washed into the hole. The clay will be allowed to settle for the next 1-2 days and then tools will be run into the hole to once again gauge TD and determine if any more clay needs to be added before cementing to surface. -4:00 pm: Left location as crew cleaned up for the day. |

Cannot see down in well (as would have to be in creek) but no oil or gas seem in the waters around the well. This is 3rd of 4th time I have checked well to see if is continuing to leak and have seen no signs, other than day of complaint when water was a little higher. Will continue to monitor. |
