0655346845 2018-12-18 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. The remediation process is progressing. The affected soils near the tank and wellhead have been scraped, placed in a plastic-lined roll-off bin, and removed for disposal. Small pockets of residual product can be seen on the surface in the vicinity of the well, and will need to be addressed. An extensive rain event occurred over the previous days, so all absorbent pads and hard/soft booms appear to have been removed from the land and streams by Steve Joy of Joy Oil and Gas. Updated pictures were taken.
0812354802 2018-12-14 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. I am accompanied by ODNR Emergency Response Nate Lane and OEPA Trevor Irwin. Steve Joy of Joy Oil and Gas is also onsite. Carpers Well Service has a vacuum truck on location removing any pooling of product within Leith Run and the drainage stream. Additionally, a plastic-lined roll-off bin is in place for the contaminated soils near the tank and wellhead. An excavator and tractor will be removing the contaminated soils to be disposed of properly. Pictures were taken.

1544793185 2018-12-13 Production Wells Violations Noted On this day, I am conducting an inspection per request of ODNR Emergency Response Scott King in regard to a product release from a storage tank, causing pollution and contamination. I was accompanied by ODNR Emergency Response Nate Lane. A valve on a load-line froze and broke, releasing approximately 30 barrels of produced fluid from a single 100-barrel steel tank, near the wellhead. The product covered a surface area of approximately 10 feet by 50 feet, and entered a drainage stream, leading to Leith Run Creek, traveling approximately 2 miles downstream towards the Ohio River. I spoke with Steve Joy of Joy Oil and Gas during the inspection, and he stated that he owned and operated the well. RBDMS scanned images indicate a request for a permit number and ownership be placed with Joy Oil and Gas in August/September 1999. Remediation of the affected area has commenced. Several representatives from Heinrich Enterprises assisted in the initial call. Absorbent pads, soft booms, and hard booms have been placed in several locations downstream. Multiple personnel from different State and Federal agencies have been onsite throughout the day. Aaron Wolfe of the OEPA, Jim Wood of the BLM, Ryan Donnelly of Division of Wildlife, and several members of the Division of Forestry, and Wayne National Fire all assisted. The issue will need to be addressed immediately. Contaminated soils will need removed and disposed of properly. Pictures were taken.
