1584965752 2018-10-30 Production Wells No Violations Inspected production well on 10/30/2018. The tank battery consists of 1-210 bbl steel tank, 1-50 bbl steel tank, 1-horizontal separator, 1-drip, and 1-meter box, located within an earthen dike. Well identification is clearly displayed at the tank battery. The 50 bbl tank was noted to be rusted through in several spots, but did not appear to be in-use at the time of the inspection. The well is equipped with a pump jack with a natural gas-powered motor, and 1-meter with an enclosed drip and regulator. No problems were noted at the time of the inspection.

1824201552 2016-07-19 Production Wells No Violations Inspected production well on 07/19/2016 to follow-up on Chiefs Order #2016-175. The tank battery

consists of 1-210 bbl steel tank, 1-50 bbl steel tank, 1-horizontal separator, and 1-meter box, located within an earthen dike. The 50 bbl tank is rusted through in several spots, and needs to be removed. Well identification is clearly displayed at the tank battery. The chart in the meter box appears to show production, however the date on the chart is difficult to read, and thus the date of production is unclear. The well is equipped with a pump jack with a natural gas-powered motor. The well is not shut-in at the wellhead, indicating that it may still be producing, and may therefore be in violation of the Chiefs Order. Findings will be forwarded to ODNR DOGRMs legal staff for further action. No other problems were noted at the time of the inspection.
