0453291650 2015-08-20 Production Wells No Violations This is a plunger lift system well utilizing 1 1/2 tubing. The tank battery is shared with Permit 4869 and consists of 1 100 Bbl. tank and vertical seperator, designated for this well. There is also a 50 Bbl. plastic water tank that is shared with Permit 4869. Additionally, there is a small compressor within the tank battery site that was in operation at time of inspection. All appears to be in good condition and surrounded by chain link fence and locked gate, as is the well head. There is presently a gas meter connected to this well, apparently for domestic use. There are no apparent problems at this time. Well does not appear to be producing. (1003-1007 Tank Battery, 1010-1013 Well.)
0888330613 2018-06-15 Production Wells No Violations Inspected production well on 06/15/2018 at the request of Supervisor Terry Whitehair to follow-up on Chiefs Order #2014-459. The tank battery consists of 2-100 bbl steel tanks, 1-1,700 gal plastic tank, 1-vertical separator, 1-horizontal separator, 1-drip, 1-meter box, and 1-compressor, located within an earthen dike; the diked area is located within a locked metal fence. The tank battery is used by multiple wells (2); well identification is clearly displayed for all wells utilizing it. A Do Not Operate tag was noted on the electric box for the compressor, however most separator valves were noted to be open, and the chart present in the meter box was current. The well is a rabbit well, equipped with a meter with 2-regulators, located within a locked metal fence. Well identification is posted at the wellhead as required, but it is very faded. A Do Not Operate tag dated 05/09/2018 was also noted at the wellhead. At the time of the inspection, it was noted that the flowline to the meter was open, however all other wellhead valves were noted to be shut-in. No other problems were noted at the time of the inspection.

The well is a rabbit well, equipped with 1-meter with 2-regulators, and located within a locked metal fence. Well identification is posted at the wellhead as required. The Do Not Operate tag noted last spring remained posted at the wellhead, but the flowline was noted to be open. A phone call to Future Production was un-returned as of 05/06/2019. No other problems were noted at the time of the inspection.

A voicemail was left with the operator regarding this issue, and a follow-up inspection will be performed promptly to ensure that the issue has been addressed. No other problems were noted at the time of the inspection.
