0237534582 2016-12-28 Casing String Witnessed No Violations Visited location on 12/27/2016 to witness the conductor casing cement job at the Morton Salt #62 well; the cement job extended from 12/27/2016-12/28/2016. A 24 hole was drilled to 130, and 118 of 18 conductor casing was run downhole. Rigged up Petroset, broke circulation, and pumped and displaced cement to surface. Good cement was seen at surface, and there were approximately 18 bbls of cement returns. Rigged down Petroset. Will wait on cement 8 hours prior to drilling out.
0372332631 2017-01-05 Casing String Witnessed No Violations Arrived on location on 01/05/2017 to witness the long string cement job at the Morton Salt #62 well. The curve was landed at 3,088 and 9 5/8 casing ran to 3,051. For the first 21 joints, centralizers were ran every 3 joints, and then every 5 for the remaining joints. Rigged up Petroset. Broke circulation with freshwater, then began pumping cement. Dropped plug and continued pumping. Plug landed at 9:00 PM. No cement was returned to surface. Pressured well up to 1,100# at 9:03 PM for 15 minute MIT. At 9:18 PM, the pressure remained at 1,100#. Long string MIT passed. As a bond log is required after drilling, the decision was made to finish drilling the lateral, and then determine a plan forward after logging the well and determining the top of the long string cement. Rigged down, moved out Petroset. Will wait 20 hours for cement to set up, and then will resume drilling operations.

Will run 1 joint of 1 tubing and grout the annulus after tubing is run and the rig has moved out.

Ran several joints of 2 plastic tubing down hole until it sat down. Pumped fresh water ahead to circulate water out of the annulus, then grouted the annular space to surface. Good cement was seen at surface. Rigged down, moved out Petroset and J-Hawk.

After scouting out the lease road upon Petrosets arrival, it was determined that lease road conditions were too poor to attempt to move in the cement trucks without a dozer. The job will be postponed until either the weather or lease road conditions improve.

1732393927 2016-12-22 Administrative Inspection No Violations Visited location on 12/22/2016 at the request of Andrew Adgate to evaluate the earthen sound barrier mound constructed along the eastern edge of the location. The mound is approximately 12-15 high, extending north-south along the entire eastern edge of the location, approximately 500 from the homes located to the east. Photos were also taken and provided to Andrew for his evalutation also. It was determined that the earthen mound is a sufficient sound barrier. Rig move in will proceed over the holiday weekend.

Good cement was seen at surface, and there were approximately 39 bbls of cement returns. Rigged down Petroset. Will wait on cement 8 hours prior to pressure testing and drilling out.
1866203405 2017-01-03 Drill / Deepen / Reopen No Violations Visited location on 01/03/2017 to check on the status of operations at the Morton Salt #62 well. HAD Rig #249 was drilling ahead in the curve at 2,802.
1887179992 2017-06-15 Preliminary Restoration No Violations Visited location on 06/15/2017 with UIC Geologist Dominique Haneberg-Diggs to approve the preliminary restoration for the well. All drilling equipment and debris have been removed, pits closed, and production lines laid to the well. Preliminary restoration passes.
