0086533198 2018-04-02 Plug / Plug Back No Violations Visited location on 04/02/2018 to witness plugging operations. Hagen Well Service and Appalachian Well Surveys were rigged-up on location, running downhole with a bridge plug. Several tight spots were worked through on the way downhole, but the bridge plug made it to 6,260. Attempted to set the bridge plug, but it wouldnt set. Began pulling out of the hole, again working through tight spots, however tools got hung in the hole at 3,095. After pulling on it, the wireline came free, leaving the bridge, setting tool, and an unspecified amoutn of wireline downhole. Rigged down, moved out Appalachian Well Surveys. Will work with operator and DOGRM staff to determine a plugging plan forward.
0297113805 2018-04-05 Plug / Plug Back No Violations Arrived on location on 04/05/2018 to witness plugging operations. Hagen Well Service was rigged-up onsite tallying tubing and preparing to run in the hole with a wireline spear. Sat down at 3,080. Attempted repeatedly to fish lost tools with the wireline spear, but were unsuccessful. Tripped out of the hole and laid down the wireline spear. Picked up an overshot tool, and ran downhole to attempt to retrieve lost equipment. Several attempts were made, but were unsuccessful. Will trip out of the hole with the overshot tool and run back downhole with tubing to set a plug.

After making repairs to the air package, Noah Horn Well Drilling Rig #831 was drilling the 7 7/8 hole ahead at 3,490. Location and pit levels look good, and installed erosion control measures (silt fencing and staked straw bales) are performing adequately. No problems were noted at the time of the inspection.

Some rough-grading has been completed, but large piles of stone and topsoil remain on location. No problems were noted at the time of the inspection.

1287529967 2018-02-16 Casing String Witnessed No Violations I was on location with Allie Oprzadek to witness the running and cementing of the 11.75 surface casing string. Noah Horn Rig # 831 was the drilling contractor and drilled a 14.75 borehole from 80 to 365 on a fresh water system. A large water flow was encountered just under the 16 conductor casing at approximately 82. The water flow impeded the penetration rate due concerns of overflowing the pits. Approximately 1200 barrels of fresh water was haul by S & H Water Service. The 14.75 borehole reached a total depth at 366 KB. Noah Horn Drilling lowered eight (8) joints of 11.74, 42 lb./ft., R-3, ST&C casing to 337 and connected one (1) 11 landing joint. Petroset Cementing Services ( PCS ) connected to the casing and conditioned the borehole with 500 lb. of gel and 320 lb. of cottonseed hulls ahead of the slurry. PCS mixed a light scavenger slurry of approximately 10 barrels to address the water flow at 82. The cement slurry weight was increased to 15.6 ppg for the remainder of the job. The cement load consisted of Class A cement blended with 3% CaCl. The cement slurry was displaced to approximately 310 using 36.5 bbl. of fresh water. I witnessed cement returns to the surface with four (4) barrels of displacement left. The casing test was conducted after the wait on cement time expired and was passed.
1384220122 2018-01-02 Casing String Witnessed No Violations Arrived on location on 01/02/2018 to witness the spudding and drilling/casing of the well. HAD Rig #618 was rigged-up onsite, and concentrically drilling and casing the conductor hole. 77 of 16 casing was set in a hard gray shale.
1400646700 2018-02-16 Casing String Witnessed No Violations Arrived on location on 02/16/2018 to witness the surface casing cement job. Drilling operations were shut-down due to lack of pit space. Waited on S&H to suck-down the pits prior to circulating the hole and tripping out drill pipe. Ran out of available working hours to witness the job due to unanticipated delays on-site. Inspector Bob Roush witnessed the cement job.

1493098917 2018-04-17 Preliminary Restoration No Violations Arrived on location on 04/17/2018 to witness pit solidification. Palmer and Sons Excavating was on-site with a dozer. Most of the water had been sucked out of the pits. 2-24 ton loads of limestone kiln dust were pumped into the pits, and stirred in with the dozer to mix. While on location, the top of cement was also checked and noted to be ~7 down from the top of the wellhead. No problems were noted at the time of the inspection.

Noah Horn Well Drilling Rig #831 was drilling ahead at 4,580. Erosion control measures are still performing adequately, and no hydrocarbon odor was noted on location. 1523449207 2018-04-11 Plug / Plug Back No Violations Upon arrival on location I was accompanied by Inspector Jeff Adams, we found the cement top at 7 feet in the wellbore. There are no signs of any bubbling in the water. Once the location is reclaimed the casing can be cut off and if needed dry cement can be used to top of wellbore.
1531796268 2018-04-04 Plug / Plug Back No Violations Arrived on location on 04/04/2018 to witness plugging operations. Hagen Well Service was on-site with a string of 2 3/8 upset tubing to fish with, as well as a wireline fishing tool. The tool was not rated to withstand anticipated pulling weights, so the crew left location to swap out tools. Arrived back on location and ran downhole with a wireline spear on 2 3/8 tubing. Will run tubing to ~3,000 and shut-down operations for the day.

1568230071 2018-03-12 Drill / Deepen / Reopen No Violations Arrived on location on 03/12/2018 to check on the status of drilling operations. Noah Horn Well Drilling Rig #831 drilled to 6,145 and the mud pump went down. Tripped ~2,000 of drill pipe out of the hole, then shut down operations to wait on repairs. No problems were noted at the time of the inspection.
1640425168 2018-02-22 Drill / Deepen / Reopen No Violations Arrived on location on 02/22/2018 to check on the status of drilling operations. Noah Horn Well Drilling Rig #831 had drilled to 4,270 and tripping out to swap the hammer for a PDC bit. Erosion control measures on-site are working adequately to control drainage from location. Two, ~6 mounds of dirt had been installed between the pit and the nearest landowner to prevent odors from leaving location. No problems were noted at the time of the inspection.
1796372674 2018-02-26 Drill / Deepen / Reopen No Violations Arrived on location on 02/26/2018 to witness fishing operations. The 7 7/8 hole was drilled to 5,252, and the bit got hung in the hole at ~5,207. Backed-off the drill pipe and ran in with jars to attempt to free-up the bit, but it was unsuccessful. KLX Energy Services ran downhole to perforate the drill pipe above the bit with 4 shots to attempt to circulate cuttings out of the hole, but it was unsuccessful. Made a second attempt further up the drill-string, and then the kelly hose blew. Shut-down for rig repairs. Will continue attempting to free-up the bit and drill string.

The location has been mostly rough-graded, however some piles of topsoil and stone remained. The wellhead is still exposed, and no change in the top of cement was noted. No problems were noted at the time of the inspection.
1946469319 2018-02-27 Plug / Plug Back No Violations Arrived on location on 02/27/2018 to witness the well being plugged back due to the bit being hung in the hole. Waited on the rig to finish running drill pipe to 4,620. Rigged up Petroset, and pumped a 750 kick plug. Circulation was established prior to pumping cement, and maintained throughout the job. No problems were noted during cementing operations.
