Noah Horn Well Drilling Rig #831 was on-site rigging up their air system and preparing to run in the hole with the surface assebly. Pits have been lined, the location bermed, and erosion control measures were in place. No problems were noted at the time of the inspection.
0516269012 2018-03-27 Casing String Witnessed No Violations Arrived on location on 03/27/2018 to witness the pressure test on the surface casing. Pressured up to 510# and held for 30 minutes. At the end of the test, the pressure had dropped 20# to 490#. Test passed.
0890354619 2018-03-28 Casing String Witnessed No Violations Arrived on location on 03/28/2018 to witness the intermediate casing cement job. Inspector Randy Akins was also on-site. Noah Horn Well Drilling Rig #831 was finishing-up running 8 5/8 casing to 1,064 with 4-centralizers. Rigged up Petroset. Pumped ~70 bbls of freshwater ahead, then started mixing gel and hulls. The well began circulating ~20 bbls into pumping cement. Good cement was seen at surface, with ~25 bbls of cement circulated to the pit. Landed the plug at 450#. Pressured the well up to 950# and held for 5 minutes. At the end of the test, the pressure had dropped to 900#. Pressure test passed. No problems were noted during cementing operations.
0897056290 2018-03-28 Casing String Witnessed No Violations Arrived on location on 03/28/2018 to witness BOP testing. B&B Oilfield Equipment was on-site rigged-up to perform the test. To test the blind rams, the well was pressured up to 500# and held for 30 minutes. No pressure was lost at the end of the test. Blind ram BOP test passed. Ran in the hole with several drill collars prior to testing pipe rams. Pressured up initially, and several leaks were noted at the BOP. Repaired BOP connections, then pressured well up to 515#. After 30 minutes, the pressure had dropped to 499#. Pipe ram BOP test passed. No problems were noted during BOP testing.

1143720507 2018-04-27 Completion Testing No Violations Arrived on location on 04/27/2018 to check on the status of flowback operations. The well was plugged off with perf balls with the tubing at 5,490. Ran downhole with swab tools to free-up the downhole obstruction, then ran tubing downhole to continue swabbing the well. No problems were noted during completion testing operations.
1204751873 2018-03-27 Casing String Witnessed No Violations Arrived on location on 03/27/2018 to witness the surface casing cement job. Inspector Bob Roush was also on-site. Noah Horn Well Drilling Rig #831 was tripping out of the hole with the last of the drill pipe. 16 joints of 11 3/4 casing was run to 598 with 4 centralizers. Rigged up Petroset. Established circulation, pumped gel and hulls ahead, then pumped cement. Good cement was seen at surface, and there were ~32 bbls of cement returns. No problems were noted during cementing operations.
1326462124 2018-04-26 Completion Testing No Violations Arrived on location on 04/26/2018 to witness the acid job on the well. Hagen Well Service was rigged up on-site. Rigged up Producers Service Corp. and waited on the water truck. Rigged up S&H Water. Broke circulation, then spotted 1,000 gal of HCl and displaced it with freshwater. Pulled 3 joints of tubing, then continued pumping to break down the formation at 1,828#. Cleared casing of acid, then shut-down operations to wait on the water truck to reload. Perf balls were released every 15-20 gal, then displaced with freshwater, and the well shut-in for 15-minutes. Rigged down, moved out Producers Service Corp. Hagen Well Service will re-run the 3-joints removed during completion testing, and prepare to swab the well. No problems were noted during completion operations.
1487436421 2018-04-30 Completion Testing No Violations Arrived on location on 04/30/2018 to check on the status of flowback operations. Hagen Well Service was on-site flowing back the well into steel tanks. No problems were noted at the time of the inspection.

1539691414 2018-10-09 Preliminary Restoration No Violations Inspected production well on 10/09/2018. The tank battery consists of 1-210 bbl steel tank, located within an earthen dike; 1-horizontal separator with a drip is located within a separate earthen dike nearby. The well is equipped with a threaded wellhead with a flanged master valve, and flowline. Well identification is clearly displayed at both the tank battery and the wellhead. Pits have been closed, the location restored, and the well placed into production. Preliminary restoration passes.
1614546894 2018-10-02 Production Wells No Violations Arrived on location on 10/02/2018. Hagen Well Service was rigged-up onsite, swabbing the well. After 8 swab runs, the well kicked-off. No problems were noted at the time of the inspection.
1665126456 2018-04-23 Drill / Deepen / Reopen No Violations Visited location on 04/23/2018 to check on the status of reclamation efforts prior to moving-in the service rig for completion operations. All drilling-related equipment (with the exception of one steel water tank) has been removed from the location. Pits are still open and fencing up, but most of the fluids have been removed. Service rig is anticipated to arrive on Wednesday (04/25/2018). No problems were noted at the time of the inspection.

Rigged up Hagen Well Service and Appalachian Well Surveys. Ran downhole with logging tools. Top of cement was noted to be at ~5,270 (~1,100 above the top perforation). Laid down logging tools and ran downhole with perforation guns. 3-gun runs were made to perforate the well from 6,376-6,420. Rigged down, moved out Appalachian Well Surveys. Hagen Well Service will run 2 3/8 tubing downhole in preparation for tomorrows acid job. No problems were noted during completion testing operations.
