1156263182 2006-08-22 Orphan No Violations On site by request of the landowner. The well has apparently started leaking around the out side of the casing and oil has found its way into field tile. The oil went to a catch basin and in to the middle branch of the portage river. The oil has gone approx 100 foot to the north and covers the water. Put two booms across the water way and called to make arrangements to have a vac truck come to the site and start clean up. Notified Dave Hodges and the OEPA in Bowling Green. Took 8 photos.
1156433551 2006-08-22 Orphan No Violations Received call from inspector Sam Faust about oil spill in tributary to Portage River. When arrived on site, Sam had contained oil in tributary to Portage river by 2 sets of absorbent booms. Oil was discharging into the creek from a drainage tile. A small but steady stream of oil from a field tile drained into a basin which had a culvert to the creek. Oil in the creek was contained from the culvert outlet to approximately 200 feet down stream. The heaviest concentration of oil was from the culvert to the 1st set of booms, approximately 100 feet in length. This area was black with oil. The creek was approximately 20 feet in width and due to dry conditions, stream flow was low. The next section of stream approximately 100 feet in length to the last set of absorbent booms had patches of open water and oil. Small traces of oil were noted escaping below the last boom. Straw bales on site were placed across the creek below the absorbent booms by Sam and I. A vacuum truck arrived about a half hour later. Oil was first vacuumed from the heavy concentration in the creek above the 1st booms. Oil was next removed from the catch basin to the culvert. Ohio EPA inspector David Schilt arrived around that time. Additional absorbent booms and absorbent pads were placed at the culvert outlet.. A hard boom was placed downstream below the last set of absorbent booms/straw bale dike. The absorbent booms and straw bales were on site from a previous land surface spill on 7/28/06 from an abandoned oil well. The oil on the ground surface had been cleaned up by Bradner Oil. Dave Barndt of Bradner Oil arrived on scene and additional measures to contain and clean up the new spill into the creek were started. There was no new oil on the surface at the site of the well where the ground spill occurred on 7/28/06. Several newspapers and the landowner, Mr. Kern were also on site .
1156796469 2006-08-23 Orphan No Violations When arrived on site at approximately 8:00am, employees from Bradner Oil were on site. Oil in the creek had been removed. Oil from the field tile was contained in the catch basin at the tile outlet. The culvert from the catch basin was blocked with plywood and the catch basin lined with heavy plastic. A trench had been excavated near the well that had caused the surface spill on 7/28/06. There were traces of oil in several spots in the trench and a field tile intercepted by the trench had evidence of oil in the tile. Later that day the outlet of the field tile that discharges into the catch basin was excavated. The tile outlet was plugged with tree roots. When the field tile was cut open, oil and water dischaged up from the hole in the tile. ODNR inspectors Sam Faust and Tom Benko were on site that day as well as Ohio EPA inspector David Schilt, Federal EPA inspector Brad Stimple, landowner Mr. Kern, Toledo ABC channel 13 news camera crew and others.
1156944472 2006-08-24 Orphan No Violations Cleanup of oil spill is now being coordinated through the Federal EPA. When arrived on site, federal EPA inspector Brad Stimple and crew from C&W Tank Cleaning Company were on site. C&W Tank Cleaning will complete cleanup of site. Discussed work done to this time. A backhoe was brought in to excavate further around the tile outlet to the creek. ODNR supervisor Jeff Fry and inspectors Sam Faust and Tom Benko were also on site during part of the day. Spoke to ODNR wildlife officer for Wood County who checked the stream. There was no apparent fish kill or other damage to wildlife from the oil spill in the creek.
1156963347 2006-08-30 Orphan No Violations Reviewed site for cleanup. Several booms are still in the creek for preventative measures. There have been approximately 2 inches of rain in the past few days. No oil noted in basin. Retention basin with siphon pipes in front of catch basin. Most of area excavated at tile outlet has been backfilled except retention basin. No oil noted in creek. Excavated hole approximately 4 feet deep at wellhead had coating of oil on water in bottom of hole. Nobody on site. Spoke to Jeff Bodi of C&W Tank by phone.
1164142131 2006-11-22 Orphan No Violations Received call from Brad Stimple of USEPA. Contractor is reported on site. Site review revealed equipment on site but no operator or activity at time of inspection. Drill rig set up on well. It appears liquid has been pumped to tank on site. It does not appear as though any rods or tubing have yet been removed from the well.
1164724119 2006-11-27 Orphan No Violations Operator was on site removing 2 inch tubing from well. The top section of the 2 inch and 4 inch casing had rusted thru allowing the remaining pipe to drop into the well. The short sections of the 2 inch and 4 inch were removed. The operator was then able to grab the remaining 2 inch string which was located at a depth of 165 feet. After removing the 2 inch, the operator will try to fish the remaining 4 inch out of the well.
1164817742 2006-11-28 Orphan No Violations Spoke to operator Ben on site. Well depth has been measured at 1310 feet deep. There was 1156 feet of 2 inch tubing removed from the well. The 4 inch casing which dropped from the surface was located at a depth of 924 feet. Attempts to remove the 4 inch casing with a fishing tool were unsuccessful. The pipe apparently was deteriorated and the tool could not attach to solid steel to lift the pipe. Rather than risk obstructing the hole with pipe fragments at the 924 depth, the 4 inch casing will remain in the well. Cement should be able to fill the area below the 924 depth. With an unobstructed hole from 924 to the surface, there will be sufficient cement to protect the fresh water aquifer above the production zone.
1165443783 2006-12-06 Plug / Plug Back No Violations Plug well. Original total depth reported at 1310 feet. Four and half inch casing dropped to base of well and top reported at 924 feet. Tubing run to 924 feet then 100 sack of Class A cement pumped into well on 12/5/06 and witnessed by KP. Top of cement was tagged at 887 feet. Tubing set at 798 feet and 9 sack grout reverse circulated to surface with 7 cubic yard load. Well filled to surface and is plugged. (This report shall be transmitted to C&W Tank Cleaning Co., Inc. Electronically and is considered signed electronically, original copy will not follow.)
1167161548 2006-12-05 Orphan No Violations Operator pumped 100 sacks of cement thru 2 inch tubing to a depth of 924 feet.. The well total depth is 1310 feet but 4 inch pipe which dropped into well obstructed tubing from going deeper. The 4 inch pipe was not successfully removed. Fluid from well was displaced to tank on site. After all cement pumped into well, the top of cement was marked at 887 feet. Since the 100 sacks of cement should fill 500-700 feet of well bore, there should be a good cement plug in the bottom of the well.
1920866944 2007-01-02 Final Restoration No Violations Well plug completed 12/6/06. All equipment has been removed from site, pit filled and site graded. Area is farm field. Final restoration will be recommended for approval.
