>>An ant colony has taken up residence in the mailbox at the site entrance containing site plans. Eviction of these tenants is recommended.

>>Sections of perimeter CFS can be removed in areas sufficiently protected by permanent stabilization.

>>Perimeter CFS can be removed in areas sufficiently protected by permanent stabilization.

>>The reported incident involved to portable toilets being blown over during windstorms on 5/2/17. No visible impact was observed at time of inspection. Affected material was reportedly removed by toilet vendor.

>>An area of containment on the pad near the site access road was partially collapsed, remediation recommended.

2626496 Primary Facility 2017-06-01 Complaint Inspection No Violations Noted On 6/1/2017 I performed a complaint inspection related to the Rostosky well site in response to complaint ID 327246. The following was observed:

>>The complainant reported excessive noise from the site every night as well as concerns about the future possibility of impact to their water supply. No current impact to their water supply was reported. The complainant stated that the overnight noise had stopped roughly a week prior but expressed concern that it would begin again.

>>Communications with the operator resulted in the operator providing contact information for their community advisor staff person and an agreement to make efforts to reduce overnight site noise when possible.
