2744772 Primary Facility 2018-06-19 Administrative/File Review In Compliance with Policy ADMIN inspection conducted in response to an operator reported release of crude oil. Department contacted operator rep Jeff Forsythe by phone. Mr. Forsythe states that around 12:00 today the well tender discovered a release of crude oil that occurred when a brine tank overflowed during a rain event. Mr. Forsythe states that the well tender reported a 2 bbl release. Based on his field observation, Mr. Forsythe believes less than 1 bbl of oil was released, not the 2 bbl originally reported. Mr. Forsythe states pads and booms have been installed to contain oil. Mr. Forsythe states this well is currently shut in and the fiberglass brine tank is not diked. Mr. Forsythe feels the release is contained and will not impact nearby waterways.

Operator appears to be in compliance with policy at this time. A follow up inspection will be conducted at a later date. Violation(s) may be issued based on follow up inspection(s).

2746471 Primary Facility 2018-06-20 Incident- Response to Accident or Event Violation(s) Noted Follow up inspection to inspection record #2744772 where an operator reported a release of crude oil.

Operator rep Jeff Forsythe and contractors on site. Mr. Forsythe states approximately 1 bbl of crude oil was released on 6-19-18 and that pads and booms were immediately used to contain the oil. Mr. Forsythe states that the site was tarped to protect area from possible rainfall. Mr. Forsythe states the brine tank will be emptied today, impacted souls will be excavated and encapsulated on site today, and soil will be removed to an Ohio landfill tomorrow morning. At time of inspection contractors on site improving access road to allow equipment access to the site. Well head and surrounding area covered with a tarp (photo #1). This tarp was removed to allow Department access to the area of release. Well tag present and legible. Oil pads and booms are installed around well head and brine tank (photo #2). The well head sits in a natural depression and it does not appear that any oil left the well site. Departme

2762252 Primary Facility 2018-08-08 Compliance Evaluation No Violations Noted CEI to inspection record #2746471.

At time of inspection brine tank has been removed. Area of release/disturbed area appears seeded and mulched (photo #1). Vegetation beginning to grow. It appears impacted soil has been removed from site. Violations issued on 6-20-18 have been corrected. Violation(s) may be issued at a later date if it appears remediation efforts were not sufficient (stressed/dead/minimal vegetation observed).

2764876 Primary Facility 2018-08-14 Administrative/File Review No Violations Noted ADMIN inspection in response to an email correspondence sent to operator. Department contacted operator rep Kirk Elkin by email to inquire about operator response to NOV (inspection record #2746471) issued on 6-20-18. Department never received this NOV response. Mr. Elkin responded via email (photo #1) and attached operator response dated 7-20-18 (photo #2-3).

This violation was closed on 8-8-18.

2814026 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted This well is tagged and shut in. The annulus is shut. The well head casing and piping look ok.

I noted that after the production waste fluid collection tank was removed from this site it has been reclaimed, seeded and mulched and the vegetation is growing back in a uniform manner. There is no distressed vegetation and no staining anywhere on the well pad.

NOTE: I will follow up on the annual production and mechanical integrity status of this well.
