Department contacted operator rep Jeff Forsythe by phone. Mr. Forsythe states that around 12:00 today the well tender discovered a release of crude oil that occurred when a brine tank overflowed during a rain event. Mr. Forsythe states that the well tender reported a 2 bbl release. Based on his field observation, Mr. Forsythe believes less than 1 bbl of oil was released, not the 2 bbl originally reported. Mr. Forsythe states pads and booms have been installed to contain oil. Mr. Forsythe states this well is currently shut in and the fiberglass brine tank is not diked. Mr. Forsythe feels the release is contained and will not impact nearby waterways.
Operator appears to be in compliance with policy at this time. A follow up inspection will be conducted at a later date. Violation(s) may be issued based on follow up inspection(s). |

Operator rep Jeff Forsythe and contractors on site. Mr. Forsythe states approximately 1 bbl of crude oil was released on 6-19-18 and that pads and booms were immediately used to contain the oil. Mr. Forsythe states that the site was tarped to protect area from possible rainfall. Mr. Forsythe states the brine tank will be emptied today, impacted souls will be excavated and encapsulated on site today, and soil will be removed to an Ohio landfill tomorrow morning. At time of inspection contractors on site improving access road to allow equipment access to the site. Well head and surrounding area covered with a tarp (photo #1). This tarp was removed to allow Department access to the area of release. Well tag present and legible. Oil pads and booms are installed around well head and brine tank (photo #2). I noted that after the production waste fluid collection tank was removed from this site it has been reclaimed, seeded and mulched and the vegetation is growing back in a uniform manner.
