Drilling appeared to have been completed at the time of the inspection, and the equipment was being brought in to stimulate the well. Freshwater impoundment located onsite had 15 feet of water according to the depth stick placed in the impoundment, and it had more than the two feet of required freeboard. Noted that the majority of the disturbed soil had been planted with vegetation and had been greened, or it had been mulched so as to minimize accelerated erosion & sedimentation until the well is stimulated and the site is ready to be restored. Observed no accelerated erosion & sedimentation with respect to either the well-site or access road.
1859105 Primary Facility 2010-01-26 Routine/Complete Inspection No Violations Noted E&S plan and ESCGP-1 located onsite during site inspection. Drilling had been completed at the time of the inspection. Below grade drill pit and well stimulation pit both had more than the required two feet of lined freeboard. All implemented erosion control measures appeared to be functioning properly and no accelerated erosion & sedimentation was observed in relation to the access road or well-site. The operator should continue to monitor the site until final restoration activities are completed.
1880025 Primary Facility 2010-04-27 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Below grade drill pit and well stimulation pit both had more than the required two feet of lined freeboard. Noted that it appeared that the well bore for permit number 005-30453 had been spudded and conductor pipe had been set. Noted that the side of the access road as it approaches Wright Road and the stream crossing for the UNT to Cherry Run had begun to develop erosion ditches and transport sediment towards the UNT. The operator should repair the erosion controls relating to the access road (i.e. rock lined and erosion control matting lined ditches) implemented to minimize the generation and transport of sediment relating to the access road.

Drilling was ongoing for well 005-30453, which is located on the same well pad. Site inspection found no accelerated erosion & sedimentation issues in relation to the access road or well-site. Noted that the fresh water impoundment and below grade drill pits onsite had more than the required two feet of lined freeboard.

All erosion controls appeared to be functioning properly and no accelerated erosion & sedimentation was observed in relation to the access road or well-site. At the time of the inspection the drill pit was in the process of being closed out and the frac impoundment located onsite was noted as empty.
1929873 Primary Facility 2010-11-23 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Noted that the access road erosion controls had been enhanced with rerocking of the collection ditches, and the installation of culverts and stabilized outlets. The area where the frac pit was had been regraded. This area required temporary erosion controls to insure that erosion & sedimentation is minimized through the winter months until the spring growing season.

Brine tanks located onsite and they appeared to be in sound condition. The Department recommends increasing the height of the diked area where the brine tanks are located. There was no accelerated erosion or sedimentation observed in relation to the reclaimed well pad or the access road. It was noted that the access road erosion controls required maintenance so that the controls continue to effectively minimize erosion and sedimentation.
2101390 Primary Facility 2012-09-20 Routine/Complete Inspection No Violations Noted Drilling was completed on 10/07/09. Wells onsite appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. The Department recommends increasing the height of the diked area where the brine tanks are located. There was no accelerated erosion or sedimentation observed in relation to the reclaimed well pad or the access road.
2170539 Primary Facility 2013-05-30 Routine/Complete Inspection No Violations Noted rilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Earthen brine tank containment had been replaced with a metallic constructed containment. There was no observed accelerated erosion or sedimentation in relation to the site or access road. The site had been stabilized and the vegetative cover met the 70% uniform coverage criteria.
