1849048 Primary Facility 2009-12-09 Routine/Complete Inspection Violation(s) Noted Drilling had been completed at the time of the inspection, drill rig not located onsite. Below grade pit located onsite and appeared to be in sound condition. Pit contained drill cuttings and fluids and had more than the required two-feet of lined freeboard. Noted that the E&S plan located onsite was thin in that it had only plan drawing, and three typical pages. The E&S plan should also include information regarding soils and soil characteristics, in addition to details as to how the drill cuttings will be disposed of. Additionaly the plan should include information on how the site will be restored. Noted that the filter fabric fence had not been installed properly.
1953865 Primary Facility 2009-12-30 Compliance Evaluation No Violations Noted Drilling had been completed at the time of the inspection. Below grade pit had been backfilled, and the site was in the process of being regraded. The Department recommends during future site constructions that culvert inlet & outlet construction include a stabilized inlet area (i.e. clean durable rock) and the outlet be constructed to minimize sediment transport during well development activities. Once these activities are completed the rock can be left or removed in favor of a vegetative stabilized inlet and outlet, as would be appropriate in this setting. The operator should continue to monitor the well-site and access road to insure all erosion controls are functioning properly until final restoration activities are completed.
1953866 Primary Facility 2011-03-02 Site Restoration No Violations Noted Drilling was completed on 12/07/09. Well appeared to be equipped for production. Observed that the well-site did not have vegetative cover meeting the 70% uniform coverage criteria nor was there another BMP implemented capable of resisting accelerated erosion & sedimentation. The well-site itself was developing erosion ditches, however there was no evidence of off-site transportation of sediment. At this time the Department is requesting documentation from the operator outlining a plan of action to achieve the 70% uniform vegetative cover on the well-site to prevent the existing erosion issues from developing into a larger problem resulting in off-site sediment transport. This documentation should be forwarded to WQS Gregory Higgins at the above address and should be received no later than March 24, 2011.
