1888023 Primary Facility 2010-05-28 Routine/Complete Inspection No Violations Noted Met onsite with operator representative Brian Welsh. Mine discharge eminating from Margaret 11 Mine Vent as a result of ongoing drilling of the above listed well was observed. After the most previous occurrence Penn DOT had dug an earthen ditch out to carry the water to drainage culvert, under Cherry Run Road and into Cherry Run. No adverse effects were noted in Cherry Run except for a small amount of cloudiness in the water which dissipated 20 to 30 feet downstream. Abundant aquatic life was observed in the stream, above, below, and in the area where the mine discharge water was entering Cherry Run. There were no dead fish, frogs, or other aquatic life was observed while onsite. No decrease in the amount of flow from the discharge was observed while onsite. Discharge area will be evaluated the beginning of next week to evaluate whether the discharge is ongoing and if any permanent erosion control measures are required.

1894824 Primary Facility 2010-06-23 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Site inspection performed as a follow-up to a drill pit fluid discharge from 06/21/10. Met onsite with EQT representative Brian Welsh. Approximately 2/3 of the contaminated media had been excavated and removed at the time of the inspection. Contaminated media was being contained onsite in covered roll-off boxes. After the excavation of the contaminated media is completed the area will be seeded and mulched. Observed no further discharge of the drill fluids to the UNT to Cherry Run. In addition noted significantly decreased flow in the UNT to Cherry Run. Brian Welsh anticipated completion of remediation activities by the end of 06/25/10. Observed no visible evidence of degradation in Cherry Run at the bridge for Wright Road.
1894826 Primary Facility 2010-06-25 Compliance Evaluation No Violations Noted Discharge remediation work was underway and nearing completion. The vast majority of drill cutting/fluids contaminated media had been scraped up and contained in covered roll-off boxes (12 in total). The areas which had been disturbed to remove the media had been seeded & mulched. The large rip rap located in the rock filter area had been removed, cleaned and put back into place. A small plastic lined sediment trap and rock apron leading from the rock filter to the trap had been built. This small trap is intended to capture any additional runoff which may contain any residual discharge contaminants which may not have been removed, and hold them so that they may be removed and taken or pumped back up to the drill pit. Six hay bales had been placed where the discharge entered the UNT as an additional barrier to prevent any further transport of the discharge contaminants. The E&S plan drawing located onsite was revised to show this additon and initialed by WQS Higgins and EQT representative Brian Welsh while onsite.

1898194 Primary Facility 2010-07-14 Routine/Complete Inspection No Violations Noted Well stimulation activities were ongoing at the time of the inspection. Fracing had been completed. All below grade pits had more than the required two feet of lined freeboard. Noted that the access road will require upkeep and maintenance after well stimulation activities are completed and all equipment is moved off-site. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. Noted that there did not appear any additional drill fluid contaminated runoff in association with the discharge from later June 2010. The collection basin implemented below the well-site to collect any runoff which may be contaminated with the drill fluids appear to contain normal runoff water. Aquatic life (i.e. frogs) were noted as in this collection pool as were insects. The area which had been affected by the discharge was seeded & mulched and the vegetation had begun to grow.

All erosion controls appeared to be functioning properly and no accelerated erosion & sedimentation was observed in relation to the access road or well-site. At the time of the inspection the drill pit was in the process of being closed out and the frac impoundment located onsite was noted as empty.
1919387 Primary Facility 2010-10-13 Routine/Complete Inspection No Violations Noted Drilling had been completed. Met onsite with operator representative Brian Welsh to discuss possible access road erosion control improvements. Agreed that the addition of turnouts and culverts to carry runoff from the high side of the road to the lower side and to break up the amount of water that reaches the bottom of the hill would be proper and effective measures. Mr. Welsh stated that this work will be undertaken the week of October 18th and that once the location of where the culverts and turnouts will be located is determined Mr. Welsh will contact WQS Higgins to review the updated erosion control map for construction and site reclamation.
1929876 Primary Facility 2010-11-23 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Noted that the access road erosion controls had been enhanced with rerocking of the collection ditches, and the installation of culverts and stabilized outlets. The area where the frac pit was had been regraded. This area required temporary erosion controls to insure that erosion & sedimentation is minimized through the winter months until the spring growing season.
