Inspection ID | Inspection Category | Inspection Date | Inspection Type | Inspection Result | Inspection Comments |
---|---|---|---|---|---|
1851881 | Primary Facility | 2009-12-15 | Drilling/Alteration | No Violations Noted | well site made - not yet spud/E&S ok/pit ok |
1852487 | Primary Facility | 2009-12-29 | Routine/Complete Inspection | Pending | Drilling completed at the time of the inspection. Possible erosion issues noted with respect to springs emerging from cut slope and information requested regarding drill cutting disposal. |
1859461 | Primary Facility | 2010-01-29 | Administrative/File Review | Violation(s) Noted | As of 01/29/10 the Department has not received the requested documentation outlining the evidence seen by the operator which show that the seasonal high groundwater table is at a depth which would enable the operator to encapsulate the drill cuttings. Based upon the evidence viewed during the site inspection the encapsulation of the drill cuttings in the below grade pit onsite is a violation of 78.62(a) (8). |
1877354 | Primary Facility | 2010-03-17 | Routine/Complete Inspection | Outstanding Violations - No Viols Req'd | Performed site inspection in conjunction with Department Geologist Micheal Morgart. Both Morgart and WQS Higgins arrived at the the same conclusion that there was a high seasonal groundwater table present on this site which would not allow for the disposal of drill cuttings via encapsulation or landfarming. |
1877355 | Primary Facility | 2010-03-18 | Routine/Complete Inspection | Outstanding Violations - No Viols Req'd | Site inspection preformed to take pictures outlining the onsite conditions at the time of the inspection to show the flows of springs emerging from the cut slope area of the well-site across the site and eventually off-site. |
2149828 | Primary Facility | 2013-03-19 | Routine/Complete Inspection | No Violations Noted | Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion related to the access road. There was an erosion issue that has begun to develop related to the collection ditch on the backside of the well pad which conveys runoff and spring flow. The outflow over the fill slope of the restored well pad has begun to cut and erosion ditch. The Department recommends that the operator implement an additional erosion control (i.e. a rock lined ditch) to effectively and stabily convey this runoff and spring flow off the well pad. The Department requests that the operator forward documentation stating that this issue has been addressed no later than April 2, 2013. |
2256299 | Primary Facility | 2009-12-15 | Routine/Complete Inspection | No Violations Noted | Site constructed drilling had not commenced. Noted filter fence installed improperly (not joined as outlined in E&S manual); no accelerated erosion observed on well site but implementation of fence could develop into a greater erosion control issue. No acc. E&S noted on access road, however operator should rerock road prior to moving rig onsite to further min. amount of sediment generated while moving rig onsite. Culvert placed in access road had hay bales placed at the outlet, however runoff then flowed onto disturbed area negating erosion control the hay bales might have provided; this installation does not agree with guidelines outlined in E&S manual. Noted in E&S plan onsite: that drill cuttings would be encapsulated and soils information included Cavode Silt Loam with SHGWT rising to w/n 6-18 inches from surface during wet periods of year which procludes disposal of drill cuttings by encapsulation/land farming. Utilization of below grade pit only possible during dry periods of year due to req. of pit bottom at least 20 in. above SHGWT. |
2284789 | Primary Facility | 2014-07-03 | Routine/Complete Inspection | No Violations Noted | Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion related to the access road or well-site. The erosion issue outlined in the 03/19/13 inspection had been addressed and remedied. |
