Information per drillers log and cement tickets: 926' of 20 lb/ft 7" casing set in 990' of 8 7/8" hole and cemented with 200 sacks of "A" containing 3% CACL2 & 1/4# flake (yield=1.18 cu ft/sk). Cement returns to surface realized with plug landing at 14:24 01-10-11. On 01-11-11 tested BOP at 350 psi for 15 min.
1949845 Primary Facility 2011-01-27 Routine/Complete Inspection No Violations Noted Well is drilled. Site ID sign and E&S plan located. Pit is on-site / appears to be in sound condition. There is no evidence of accelerated erosion and sedimentation on the well site or the access road.

Brine tanks located onsite and they appeared to be in sound condition. Site had been regraded, and the fill and cut slopes had vegetation which met the 70% uniform coverage criteria. There were several small erosion ditches observed on the fill slope side of the well pad. The Department is requesting that the to address this issue the operator scarify the unvegetated portion and seed & mulch the remainder of the well pad to alleviate the runoff causing the minor erosion.
