2042194 Primary Facility 2012-02-07 Complaint Inspection No Violations Noted On February 6, 2012, the Department received a complaint regarding Chesapeake¿s access road to their Bussard site. The complaint alleges that the site was a mess and that Chesapeake was constructing the road without a lease from the landowner. Although the Department does not get involved in land disputes I did check the site¿s condition on February 7, 2012.
2042198 Primary Facility 2012-02-13 Complaint Inspection No Violations Noted On February 13, 2011, I stopped at the Bussard site to check the progress. A new contractor, Anderson Excavating LLC was working on site and I spoke to Jim Keys the Field Superintendent. I checked the plans Mr. Keys was working from and they were identical to the ones submitted to the Department. The GPS coordinates for the access road entrance with Westwood Drive are listed on the plans as: N 40° 39¿ 37.3¿ and W 80° 29¿ 07.5¿. My GPS located the entrance at: N 40° 39¿ 37.3¿ and W 80° 29¿ 07.4¿, only a tenth of a second different from the plans.

The road was mostly in and a large (500 foot square) area was prepared for the drilling site. The work appears to have been done as presented in the plans. The Frac Pond was not yet built, and Anderson’s foreman on-site explained that he had not yet received permission to build it.

2104540 Primary Facility 2012-10-01 Routine/Complete Inspection No Violations Noted John Ley WQS, SWRO was on site to conduct a routine inspection. At the time of this inspection, the well sign, E&S plan, well permit and the PPC plan were onsite. The site was gated for security. The well was drilled. Bmp's installed were functioning according to the E&S plan. No accelerated erosion or sedimentation issues were noted on site or the access road. The fresh water impoundment was inspected and was found to be in good shape with no evidence of leakage apparent.
2185121 Primary Facility 2013-07-18 Complaint Inspection Violation(s) Noted This site was inspected in response to a complaint (#298286) concerning security of the water impoundment and storm water runoff from the well pad. The water impoundment was fenced in and the gate was locked. The chain link fence had apparently been vandalized to allow unauthorized access under the fence. Perimeter E&S controls had been removed from the site and annotated on the on-site copy of the ESCGP without prior DEP authorization. The site does not have 70% uniform vegetative cover. There was sparse vegetative cover around the berms of the water impoundment and incipient erosion trails were evident. While there is generally good vegetative growth around the perimeter of this site, off-site transport of sediment cannot be ruled out.
2199339 Primary Facility 2013-07-25 Follow-up Inspection Outstanding Violations - No Viols Req'd Inspector met with Operator's representatives at the well site. Site was drilled but not equipped for production. The impoundment's northeastern and southeastern slopes had unvegetated areas. Since last inspection, Operator had replaced super silt fence along the well pad perimeter northeast of the impoundment. Sufficent vegetated cover obviates the need to re-install other perimeter controls at the site. Department recommends Operator take necessary measures to stabilize unvegetated areas.
2200031 Primary Facility 2013-08-09 Follow-up Inspection Outstanding Violations - No Viols Req'd This inspection occurred during a period of heavy rain. Site was drilled but did not appear equipped for production. Water impoundment had necessary 2 feet of freeboard. Flash flooding had washed out access road near entrance. Unvegetated areas near water impoundment had been mulched and seeded. Culverts under access road near the water impoundment were unobstructed and conveying water to the south side of the site away from residences situated to the northeast. Operator was contacted regarding the access road and dispatched a crew to make repairs. Department suggests Operator review design of drainage ditches and storm water controls along the access road to ensure they are adequate for expected precipitation.
2202940 Primary Facility 2013-09-09 Compliance Evaluation In Compliance with Policy Trucks were on-site taking water from impoundment during inspection. Access road had been repaired since last inspection. This site was drilled but did not appear equipped for production. Well was properly labeled. Cellar of well had deep water accumulation from recent storm activity. Water impoundment had necessary two feet of freeboard. Temporary stabilization of area around water impoundment appeared to be very good with only a few small areas without vegetative growth.
2259289 Primary Facility 2014-04-01 Routine/Complete Inspection No Violations Noted This well is regulatory inactive. Site was drilled and had brine tanks and production equipment on the pad. Well was properly labeled. Water in containment around brine tanks was clear without sheen. Temporary stabilization appeared to be very good. Fabric of one silt fence was loose on the upper half of end piece; operator notified.
