Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2659822 Primary Facility 2017-11-14 Drilling/Alteration No Violations Noted 40' of 16" conductor was cemented in a 20" hole. The cellar 8'x8'.

The site is in good condition.

2659968 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined in the E&S plan and the construction sequence. Site had been temporarily stabilized and the vegetation had begun to emerge and grow. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Drilling had been commenced but was not ongoing at the time of the inspection. Work was underway to implement the secondary containment after completion of setting the conductor pipe. Additionally work was underway to install fencing around the freshwater impoundment, which is currently unlined.
2663197 Primary Facility 2017-11-21 Drilling/Alteration No Violations Noted Conductor is set at 40' and ready for rig. No violations observed. NOVIO
2670247 Primary Facility 2017-12-13 Drilling/Alteration No Violations Noted Rig and equipment demobilizing after cementing 8 5/8" casing. No violations observed. NOVIO
2670352 Primary Facility 2017-12-13 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined in the E&S plan contained within the ESCGP. All erosion controls had been properly maintained. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Drilling had commenced but was not ongoing. Top-hole rig and associated equipment was in the process of being moved offsite. Freshwater impoundment construction had been completed but the impoundment had not been lined.
2691207 Primary Facility 2018-02-02 Routine/Complete Inspection No Violations Noted Site and access road had been constructed in accordance with the ESCGP. All site and access road erosion controls had been implemented and maintained properly. There was no observed accelerated erosion or sedimentation in relation to the site or access road, however snow accumulation made an evaluation of the erosion controls limited. Earth disturbance and work related to ESX17-007-0006 had commenced. A portion of this pipeline overlaps with the well site and its erosion controls. In the area of overlap the pipeline project's erosion controls are downslope and take precedence over the site's erosion controls. Secondary containment was located onsite, drilling had commenced but was not ongoing. Well development impoundment construction had been completed. Well development impoundment had been lined and put into service. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i).
2701626 Primary Facility 2018-03-06 Drilling/Alteration No Violations Noted Patterson began drilling the bottom section of this well at 2544'. The rig has reached a depth of 6420'.i
2708053 Primary Facility 2018-03-21 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection for well 007-20510 B24-4H. Secondary containment located onsite and drilling fluid containers located onsite were within tertiary containment. Well development impoundment located onsite and it appeared to be unlined and not in use. Site and access road had been constructed in accordance with the ESCGP. All site and access road erosion controls had been implemented and maintained properly. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i).
