2694292 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Site and access road had been constructed in accordance with the ESCGP. All site and access road erosion controls had been implemented and maintained properly. With the upcoming precipitation forecasted the Department recommends the operator inspect the collection ditch in between the pad and impoundment and the rock outlet area for the sed trap to insure their continued proper function. There was no observed accelerated erosion or sedimentation in relation to the site or access road. There was an Act 9 sign located onsite in accordance with 25 Pa Code Chapter 78a.55(i). Drilling for 007-20514 Per B22 2H was ongoing at the time of the inspection. Secondary containment was located onsite and in use. 11 wheeled storage tanks were located onsite, 9 of which were located within tertiary containment. Additionally the vast majority of other containers located onsite were also within tertiary containment.

2772213 Primary Facility 2018-09-07 Follow-up Inspection In Compliance with Policy Follow up inspection related to a diesel spill reported on 9/6/2018. The Operator contacted the Department on 9/6/2018 to report a release of diesel fuel to the well pad. The Operator indicated that a truck hauling acid backed into the sand storm 5 container, located on the south west corner of the well pad, which punctured the fuel tank causing the release to the well pad. The estimated diesel fuel spilled was approximately 20 gallons based on the max capacity of the fuel tank and what was removed from the tank after the release. The Operator indicated that approximately 50 gallons was removed from the tank, leaving approximately 20 gallons unaccounted for. Based on the Department inspection and conversation with Joe Schwab during this inspection the Department requests additional information pertaining to the amount of fuel spilled based on the conversations regarding the amount of diesel fuel unaccounted for.
