Met with Chesapeake staff including: Eric Haskins and production engineers Max Goff, Lauren Tenpenny and field operations engineer John Turrin for a pre-plugging inspection. The Sullivan 1, spudded 8/2008, targetted the Trenton/Black River formation, with a TD 14,617’. When H2S was found in the formation gas (nearly 1%) the well was killed with fluid and shut-in. In 2014, Chesapeake isolated the H2S-bearing Black River formation with a cemented cast iron bridge plug (12,300’). Quarterly monitoring since that time apparently detected low levels of H2S, but confirmatory tests showed these to be false positives. The well had a 3-string design. Surface casing was set at 1530’, intermediate casing at 8,280. The intermediate casing is not cemented to surface with the estimated TOC at 3,800’. One annulus, 9-5/8” x 7” was accessible, No combustible gas detected at time of inspection. No H2S was detected, and no bubbling was observed in the cellar. Chk plans to monitor sur x int.

2520388 Primary Facility 2016-09-15 Routine/Complete Inspection No Violations Noted Inspection of the Sullivan 1H (015-20082) well in Bradford County conducted September 15, 2016 at 1:00 pm. I received some documentation relating to the plugging of this well from another inspector early this morning and went to the pad to check the status of this well. At the time of inspection; No equipment or personnel on site. The well has been plugged and buried with no monument or vents at the surface. I was able to find an 8’x8’ well cellar with my metal detector where the well is buried at these coordinates; 41.92900, -76.57872. The well was drilled in ’08 to TD of 14,617’ (Trenton Black River shale), and temporaily P&A’d with CIBP & cement spotted on top. The documentation I received looked like the operator was proposing to perforate the 7’ production casing above this plug and attempt a circulation squeeze to bring TOC closer to 9 5/8” shoe rather than cut and pull max amount of 7’ due to foreseen difficulty given the overall depth of the well (would have to pull ~11,250’ of 7” 29#/ft csg). 2527188 Primary Facility 2016-10-12 Plugging(Includes Plugged/Mined Through) No Violations Noted On October 12, 2016 at approximately 11:00 am I arrived at the Sullivan 1H well site after reviewing the Plugging Certificate recently submitted by the operator. Chesapeake was on location where they had a crew seeding the pad. The well has buried approx. 5’ below ground level after plugging with a marked conductor cap. A photo of the marked well was submitted along with the plugging certificate. I met with Chesapeake employees to help me find the buried well. It turns out the metal 8’ x 8’ cellar I found during my last visit 9/15/16 was a cellar from a well that never materialized I was informed. According to the employees on location the Sullivan 1H was buried approx. 15’ north of this location. I used a metal bar to make a 1/8” hole approximately 4’ deep to probe the ground with rod and hose rigged up to my Altair 5x gas meter and monitored for 5 minutes and found no gas. I signed the Plugging Certificate after reviewing description of work related to the job but was not on location to verify no surface
