2576204 Primary Facility 2017-03-28 Routine/Complete Inspection No Violations Noted On March 27, 2017 at 1030 hrs., the department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Otis Well Pad location in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A Chesapeake representative was on site performing routine maintenance during the inspection.

The E&S plan, site sign and weekly inspection logs were on site and available for review. The slopes of the well pad were observed to be well vegetated. No erosion issues were observed. The Department observed two infiltration basins installed at the Otis Location.

There were two wells in production phase during the inspection. Both wells were labeled and no bubbling was observed in the well cellar. Two production tanks and two GPUs were also observed on site. There was no staining observed on the well pad and all puddles tested low in specific conductivity.

The ESCGP-2 for the Otis Location expires on 3/29/2020. PCSM has been addressed at the site.

2656362 Primary Facility 2017-10-30 Site Restoration No Violations Noted On October 30, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Otis Location in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake. A Notice of Termination was received by the Department on October 25, 2017.

The Department observed rutting/ disturbed earth at the southwest corner of the well pad. The rutting was caused by mowing activities on the slopes of the impoundment. Because of the disturbed earth, the site does not meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. The rest of the site was observed to be well vegetated.

The well pad was not reduced in size, but two infiltration areas were observed to be installed as per the approved ESCGP-2. All temporary E & S BMPs were observed to be removed from the site. Based on conditions observed during the inspection the Department recommends denying the Notice of Termination.

2750126 Primary Facility 2018-07-10 Site Restoration No Violations Noted Two wells staged for production phase. Two each sand and water separators. No leaks detected from production equipment. No violations noted.

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 7/3/2018. Based on this inspection the Post Drilling Restoration report was accepted on 7/10/2018.
