2461174 Primary Facility 2016-03-04 Routine/Complete Inspection No Violations Noted
2569152 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted One well in production phase. One each sand and water separator. No leaks detected from production equipment. Site stable. No violations noted.
2820802 Primary Facility 2018-12-27 Site Restoration No Violations Noted A Post Drilling Site Restoration Report was submitted to the department on November 8, 2018 which promoted this inspection. One well is staged for production. One GPU and sand separator is present without containment. One double-walled fiberglass produced water tank is present and is in good condition.

A permitted stream crossing is present along the access road. Along the downstream side of the stream crossing, significant erosion is present. A tub has formed just down stream of the culvert pipe, rock material has been moved downstream, and sections of E&S matting has been destroyed.

A cut slope is present on the northeast side of the pad. Two sections of rip rap are present on the slope. According to the E&S log on site, one of these sections of rip rap was recently added. Where this new section of rip rap is, the slope is continuing to slide above the rip rap and on the upper right side. The E&S log notes this sliding as well.

The pad side slopes are well-vegetated with greater than 70% vegetation. T
