2078933 Primary Facility 2012-06-12 Routine/Complete Inspection No Violations Noted Inspection conducted in response to Chesapeake’s request to discharge rain water which has accumulated on the pad due to ponding conditions. Original request was to open a hole in the berm, allow water to flow down a rip rapped ditch, travel over a level spreader, and then flow through a well vegetated field. During inspection, found that there was an extremely high volume of water on the pad, that pipeline had recently been installed below the pad, which happens to pass right below the above mentioned level spreader. The pH of the standing water on the pad was 8.0 and the conductivity on the water was 650 uS/cm. Hence, technically OK to discharge this water. However, because of the volume of water and location of the unstabilized pipeline ROW, Chesapeake needs to develop a comprehensive plan to discharge this this water. I contacted Eric Hottenstein and told him my findings, and also stated they needed to submit a plan for this proposed discharge. I also contacted Matt Stetter of AMS make him aware of the pipeline conditions.

2592745 Primary Facility 2017-05-11 Routine/Complete Inspection No Violations Noted On May 11, 2017, at 1145 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Treat Location in Rome Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The Department could not access the lock box that house the E & S plans because the lock would not open. The site was well vegetated and no erosion issues were observed. An infiltration basin was installed in 2016 and did not yet meet the 70% vegetation requirement.

Two wells were connected to production during the inspection A third well was drilled but was not connected to production. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and three GPUs were on site. The secondary containment for the production tanks was filled about two-thirds from the top of the containment with storm water. The Department recommends removing the storm water from the containment. All puddles were low in spe

2690091 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted On January 18, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Treat Location in Rome Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was snow covered. PCSM has been addressed at the site with the installation of an infiltration berm. The well pad has not been reduced in size.

Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks were on secondary containment. Two GPUs were also on site. The pad was snow covered so no conductivity readings were taken. No staining was observed.

No violations were observed during the inspection.
