2662941 Primary Facility 2017-11-15 Site Restoration No Violations Noted On November 15, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Katzenstein Location in Wysox Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Generally the well pad location was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. No erosion or sedimentation issues were observed. The well pad has not been reduced in size, but PCSM has been addressed.

Two wells were in production during the inspection. Both wells were labeled and no bubbling was observed in the well cellars. The GPUs and two production tanks were also on site. Puddles on the pad were low in specific conductivity. No staining was observed on the well pad surface.

Based on conditions observed during the inspection, the Department recommends approving the Notice of Termination. No violations observed

2726026 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted Two wells staged for production phase. Two each sand and water separators. No leaks detected from production equipment. No violations noted.
