2582151 Primary Facility 2017-04-10 Site Restoration No Violations Noted On April 10, 2017, at 1230 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC Jacobs Location in Rome Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. An employee for Chesapeake was on site performing maintenance work on the GPUs and the production tanks.

The E&S plan, site sign and weekly inspection logs were on site and available for review. The site was observed to be well vegetated. No erosion issues were observed. An earthen containment berm was observed around the perimeter of the pad. PCSM has not been addressed at the site.

There were two wells in production during the inspection. The wells were labeled and no bubbling was observed in the well cellar. Two GPUs and two production tanks were observed at the site. The production tanks were observed to be in secondary containment. The containment structure appeared to be in good condition. No staining was observed on the well pad surface. 2685154 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted On January 18, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Jacobs Location in Rome Township, Bradford County. The Department was represented by Andrew Hobbins. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last inspection conducted by Chesapeake occurred earlier in the day today (1/18/2018). The site was snow covered. According the the E & S plan on site, PCSM has not been addressed at the site. The well pad has not been reduced in size.

Two wells were in production phase during the inspection. The Wells were labeled and no bubbling was observed in the well cellars. There were two production tanks and two GPUs on site as well. No staining was observed on the pad surface. No conductivity readings were taken because all puddles were frozen.

No violations were observed during the inspection.
