1966567 Primary Facility 2011-02-03 Compliance Evaluation No Violations Noted
2008135 Primary Facility 2011-09-27 Routine/Complete Inspection No Violations Noted 2 wells drilled. 3 cellars. Cellars filled with water. Raining heavily. 0 psi on 9 5/8. 0 psi on 5 1/2. Small amount of fract sand on site, however contained on liner.
2130687 Primary Facility 2013-01-09 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Arrived at the site at 12:10 in response to an estimated (10) gallons of a production fluid spill. I was accompanied by Mary Maryott, EH&S Specialist of Chesapeake. Chesapeake reported the spill to the Department on 1/2/13 at 16:25. The report does not indicate the actual date and time of the spill. The incident report indicates the spill occurred as a result of a sight glass breaking on a water truck, spilling about (10) gallons of produced water on the surface of the ground on the Well Pad. An excavator dug up the contaminated soil and staged it on the Well Pad. Mary indicated to me during the inspection that the production fluid spilled on the snow. An excavator scraped the top surface of the ground where the spill occurred and placed the material which also consisted of snow and slush, in a containment system consisting of a plastic berm. The fluid on the surface was sucked up and transported to the Chesapeake’s Sharer Centralized Fluids Processing Facility for disposal and treatment.
2203625 Primary Facility 2013-09-11 Routine/Partial Inspection No Violations Noted clean and orderly - no junk or visible spills
2206276 Primary Facility 2013-09-20 Routine/Complete Inspection No Violations Noted
2581845 Primary Facility 2017-04-11 Routine/Complete Inspection No Violations Noted Inspection conducted to assess overall site conditions. As discussed elsewhere in this report, the site is not currently compliant with site restoration regulations, and corrosion of the produced water tanks was observed. No violations were cited.
2616983 Primary Facility 2017-07-03 Routine/Complete Inspection No Violations Noted
2708425 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. No violations cited.
