2500770 Primary Facility 2016-07-08 Site Restoration No Violations Noted On July 8, 2016, at 1325 hrs.,the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Foster Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present during the inspection on behalf of Chesapeake. A Notice of Termination was received by the Department on 6/21/2016.

Generally the well pad location was determined not to meet the requirements of 25 Pa Code 102.22(a)(2)(i) Permanent Stabilization. Several areas were observed to have less than 70% uniform vegetation. The Department also observed silt fence that was in place on the cut slope of the well pad location. The Department observed that the well pad had been reduced in size, but was not restored to original contours. No trash or non-essential equipment was observed on site.

Based on the conditions observed during the inspection, the NOT is not recommended for approval.

2671014 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted On December 11, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Foster Location in Wysox Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was stabilized with rock and perennial vegetation. No erosion or sedimentation issues were observed. The well pad has been reduced in size, but PCSM has not been addressed.

Two wells were in production during the inspection. The wells were labeled and water within the well cellar was frozen. No bubbling was observed. Two production tanks were labeled appropriately and in secondary containment. There were two GPUs also on site. Puddles on the pad were frozen so no conductivity readings were taken. No staining was observed on the well pad surface.

No violations were observed during the inspection.
