2587931 Primary Facility 2017-04-26 Routine/Complete Inspection No Violations Noted On April 26, 2017, at 1105 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Acton Location in Rome Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A crew of workers with Parco was on site during the inspection performing maintenance work on the well.

The site sign, E&S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were observed to be well vegetated. An infiltration area was observed to be installed off of the southeastern corner of the well pad. The infiltration area had some vegetative growth, but did not meet 70% perennial vegetation. Silt fencing was observed to be installed down gradient from the infiltration basin.

One well was in production phase during the inspection. Workers from Parco and a representative from Chesapeake were on site performing maintenance work on the well. The well was observed to be labeled appropriately. Two production tanks and on

2662927 Primary Facility 2017-11-15 Site Restoration No Violations Noted On November 15, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Acton Location in Rome Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Generally, the well pad location was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. No erosion or sedimentation issues were observed. An infiltration basin installed at the site addresses PCSM requirements.

One well was in production during the inspection the well was labeled and no bubbling was observed in the well cellar. Two production tanks and one GPU were also on site. Conductivity readings from the pad surface were low.

No violations were observed. Based on the conditions observed during the inspection, the NOT is recommended for approval.
