2507171 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted On July 27, 2016 at 1230 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Bo Well Pad Location in Tuscarora Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Chesapeake did not have a representative present during the inspection. Two employees of Meshoppen Stone Inc. were on site constructing the PCSM BMPs during the inspection.

The site ID sign, E&S plan and weekly inspection reports were all on site and available for review during the inspection. Chesapeake was granted a 2-year pad restoration extension on 1/20/2016. As part of the extension PCSM BMPs had to be installed at the site. The Department observed retentive grading/infiltration area #1 to be installed along the access road to the well pad. The area was not temporarily stabilized, but was constructed earlier in the day. The Department observed the two workers for Meshoppen Stone Inc. working on installing retentive grading/infiltration

2682995 Primary Facility 2018-01-11 Routine/Complete Inspection No Violations Noted On January 11, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC Bo Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Chesapeake last inspected the site on 1/8/2018. The site was observed to be well vegetated. No erosion or sedimentation issues were observed at the site.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. One production tank and one GPU were also on site. The tank was double walled and labeled. No staining was observed on the well pad surface. No conductivity readings were taken.

No violations were observed during the inspection.

2770514 Primary Facility 2018-09-04 Site Restoration No Violations Noted One well staged for production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted. The Operator states the well site is sized appropriately for safe operation.

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 9/4/2018. Based on this inspection the Post Drilling Restoration report was accepted on 9/4/2018.
