2229219 Primary Facility 2013-12-11 Routine/Complete Inspection No Violations Noted I met with Lee Simons of Bishop Brothers Construction of Ulster, PA. Bishop Brothers was in the process of moving and staging equipment at the site. Mr. Simons informed me the frac job was completed about (2) weeks ago. Frac plugs have been drilled out and the well has been flowed back to some degree. During well site flowback procedures, natural gas was not encountered. Talisman will rig up and resume flowback procedures in early January of 2014. Mr. Simons also mentioned all frac tanks and manifold lines on locations are empty. There were some tears in the secondary containment liner, both the felt and plastic. The surface of the ground was not revealed were the tears were observed.

2596725 Primary Facility 2017-05-22 Routine/Complete Inspection No Violations Noted On May 22, 2017 at 1100 hrs., the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC, Antisdel 05-036 well site location. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. Two areas on the cut slope were observed to be stabilized with rock. PCSM has been addressed at the site.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. One production tank and one GPU were observed to be in secondary containment. Puddles on the well pad surface were low in specific conductivity. No staining was observed on the well pad.

The pad has not been reduced in size, but PCSM has been addressed at the site. A two year restoration extension request was approved by the Department in 2016.

No violations were observed

2774874 Primary Facility 2018-09-13 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the Repsol Antisdel (05-036) L Pad in Warren Twp, Bradford County on 9/13/2018 at 1325 hours. The inspection was conducted in response to a Notice of Termination received by the DEP. The weather was cloudy, 73 degrees and 90% humidity.

The site appears to be stabilized and well vegetated. All planned PCSM practices are in place and appear to be functioning properly. No temporary E&S BMPs were observed and no erosional issues were noted. A post-drilling well site restoration report has been accepted by the DEP.

This Notice of Termination is recommended for approval. No violations are noted at this time.

I departed the site at 1400 hours.
