2807898 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted I met with Clay Westerfield, company man for Chesapeake, upon arriving on location. Clay informed me that they had begun to frac just before Thanksgiving. Well 3H is in the process of being frac'd and after they will move to the 4H well. Portions of the pad not covered with secondary containment had several inches of mud present. Secondary containment was present and constructed of felt, 125mm plastic, and plastic rig mats on the surface. Berms were present on all sides of the secondary containment. Two ASTs with plastic lining are on location and both contained fresh water. Fresh water is being trucked in and unloaded into one AST and then it is pumped to the second where it then goes to the Fountain Quell mixing truck where chlorine dioxide is added in leu of biocide.

Gaps in the earthen berms surrounding the pad have been filled prior to the start of the frac. No stormwater was seen leaving the pad. A vacuum truck is present on location to remove any stormwater that accumulates on the pad. Clay informed

2815825 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:18. The weather conditions are mostly sunny skies, breezy and brisk, with temperatures in the 30's. I met with Greg Black, site company man, and consultant of CHK. BJ Services is the CHK subcontractor in the process of conducting hydraulic fracturing (stimulation) operations. CHK submitted a stimulation notification to the Department on 11/19/18. The comments in the notification indicated CHK would be moving to the Lines Pad for frac operations on the 3H and 4H wells between 11:00 AM on 11/20/18, to 12:00 PM on 11/21/18. Mr. Black stated the following regarding site activities. "Stack frac is in progress on the Lines 4H well. The frac job is done on the 3H well. We should finish up late tonight or early tomorrow morning with the frac and start to rig down. We will go to Cannella pad next to conduct frac operations. The above ground storage tank (AST) near the pad entrance is (10,000) barrels in capacity. Freshwater is trucked in and transferred via a pump into this AST.
2816831 Primary Facility 2018-12-14 Routine/Complete Inspection No Violations Noted Inspection completed in order to investigate some possible erosion on the southeast slope of the pad. The frac is complete. ASTs are disassembled and removed from the pad. Brine tanks remain on site. Roll-offs on site. Secondary containment remains in place.

Puddles on the pad range between approximately 900uS/cm to 3mS/cm. >20 mS/cm fluid is present on containment. The department recommends removing this fluid from the pad as soon as possible, if not done already.

On the south side of the pad, a pipeline access road runs parallel to the pad at a higher elevation. Three culverts run under this access road and empty out on the slope leading to the pad. Below the center culvert, vegetation is pushed down and has some mud on it. This appears to be a result of a high volume of water coming from the culvert. It does not appear that there is erosion occurring on the slope. The rock apron coming from the culvert is intact. The slope does not appear likely to slip.
