2352393 Primary Facility 2014-11-10 Routine/Complete Inspection No Violations Noted 3 wells on pad. Unable to determine if the conductor was set on wells 1H and 2H as cellar was covered with cement and manhole. Rogers 3 well was in production, however no gauges on any of the annuli. Cellar filled with water, with no visible bubbling.
2465417 Primary Facility 2016-03-16 Routine/Complete Inspection No Violations Noted
2627754 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted On August 16, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Rogers H (05-235) well site in Windham Township, Bradford County. Andrew Hobbins, Water Quality Specialist represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. No erosion or sedimentation issues were observed. PCSM has been addressed at the site.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. On GPU and One production tank were in secondary containment. Liquid within the containment was low in specific conductivity. Puddles on the pad were also low in specific conductivity. No staining was observed.

Two infiltration basins were installed at the site to address PCSM. The well pad has not been reduced in size. A two year restoration extension was approved by the Department on 12/2/2016. No violations were observed during the

2810389 Primary Facility 2018-12-04 Site Restoration No Violations Noted NOT approval.

Electrical conductivity readings taken from pooled water with an Oakton PCSTestr 35. A puddle on the pad surface near the production tanks is 0.06 mS/cm. JLP
