2579831 Primary Facility 2017-04-03 Routine/Complete Inspection No Violations Noted On April 3, 2017, at 1400 hrs, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC Hart Location in Wyalusing Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The E&S plan, site sign, and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. No erosion issues were observed. Two sediment basins were observed to be installed south of the well pad. One of the sediment basins was filled with stormwater from recent heavy rainfall. PCSM BMPs have not been installed at the site.

One well was in production during the inspection. The well was labeled correctly. Minor bubbling was observed in the well cellar. Eric Hottenstein of Chesapeake was notified of the bubbling in the well cellar. He stated he would have someone look into it as soon as possible. On GPU and two production tanks were observed on site. The production tanks were obs

2720871 Primary Facility 2018-04-20 Routine/Complete Inspection No Violations Noted One well staged for production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted.
2825505 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:37; the weather was ~25°F and windy. There were several patches of ice and snow on the pad.

There was 1 properly tagged Marcellus well staged for production. There were 3 well cellars onsite and filled with ice. There was 1 GPU, 1 sand trap, and 2 produced water tanks.

There was a lighting unit on the pad and in secondary containment (Photo #1). There was also a section of plastic secondary containment that was torn down the middle and appeared to be of no use (Photo #1).

The areas around the pad were covered by a dusting of snow but vegetation was observed to cover more than 70% of the ground. No E&S issues were noted. There appeared to be an infiltration berm along the southeastern edge of the pad. There was a small amount of ice in the berm.

No violations noted.
