2726381 Primary Facility 2018-04-23 Routine/Complete Inspection No Violations Noted I arrived at the location at 12:11. The weather conditions are clear skies, mild with a light breeze and temperatures in the low 60's. I had a brief conversation with Brian Millage and Kyle Oetting of CHK. Mr. Millage and Mr. Oetting informed me that they are in the process of checking serial numbers on valves as part of the wells. The Claudia Bra 1H and Claudia Bra 5H wells were just recently completed. Completions activity included hydraulic fracturing (stimulation), drilling out frac plugs and flowback of these particular wells. The Claudia 2H and 4H wells have been in production for quite some time. A vacuum truck owned by CS Trucking of Dover, Ohio mobilized to the site in attempt to offload production fluid from the production fluid storage tanks. The driver stated that he will not be able to offload production fluid due to an insufficient amount of fluid in the tanks. 2727161 Primary Facility 2018-04-26 Follow-up Inspection No Violations Noted I arrived at the site at 11:05. I was accompanied by Casey Baldwin, DEP O&G WQS of the Scranton District Office. The weather conditions were partly sunny skies and breezy, with temperatures in the upper 40's to low 50's. There was no activity or personnel at the site as we arrived. Shortly after we arrived we met with Steve Tewksbury, Supervisor of Meshoppen Stone. He informed us that he has mobilized to the site to pick up trash. The intent of the inspection was to conduct field specific conductance (conductivity) screening of water on and off containment due to elevated conductivity readings I recorded during an inspection at the site on 4/23/18. The Department recorded elevated conductivity readings during the 4/26/18 inspection on and off containment near frac tanks used during completions operations and in the corner of the pad near the site perimeter berm directly across from the junction of the access road and pad entrance. 2743815 Primary Facility 2018-06-15 Administrative/File Review No Violations Noted This administration inspection report is created to document the Department's receipt of sample results related to the discovery of elevated specific conductance (conductivity) readings recorded in water on and off secondary containment during the Department's 4-23-18 and 4-26-18 inspections conducted at the site. The Department collected a sample on the pad surface as a result of the elevated conductivity readings. For additional details regarding the conductivity readings and samples collected refer to the Department's Claudia 4-23-18 and 4-26-18 inspection reports inspection ID #'s 2725060 and 2726829. The Department returned to the site on 5-16-18 to conduct a follow-up inspection. CHK had informed the Department during the 5-16-18 inspection, inspection ID # 2742626, that the pad surface was remediated in the general area where the Department had recorded elevated conductivity readings and collected the samples on 4-26-18.

2743870 Primary Facility 2018-05-16 Follow-up Inspection No Violations Noted I arrived at the site at 11:09 AM. Light to moderate rain was falling with temperatures in the low to mid 60's. There was no activity at the site during the inspection. Nick Krut and Danielle Schaad, Environmental Representatives of CHK, were at the site during the inspection. The secondary containment liner, frac tanks and gas buster tanks associated with completions operations, have been removed from the site. The containers storing impacted pad surface material associated with a spill that occurred at the site on 3-26-18, have been removed from the site as well. Mr. Krut and Ms. Schaad stated that the area of the pad surface was remediated on 5-10-18 where the Department recorded elevated conductivity readings on 4-23-18 and 4-26-18, and collected surface water samples on 4-26-18. I used a Hanna HI 9831 Soil Test EC &Temp meter, a Hanna HI 9831 Groline Soil Test meter and an Extech Instruments, ExStik 2 meter to conduct field screening for conductivity in water on the pad surface. 2745474 Primary Facility 2018-06-26 Administrative/File Review No Violations Noted This administration inspection report is created to document CHK's discovery of discrepancies in the Department's 6-15-18 inspection report for the Claudia 1H well, inspection ID # 2745290. Eric Hottenstein, Environmental Supervisor of CHK forwarded an email to me on 6/25/18 at 13:33 requesting a revised version of the inspection report. The email contained the 6-15-18 inspection report with the discrepancies highlighted in red in the remarks section of the inspection report. The Department had listed sample analysis results in the remarks section for sample #'s 4559-255, 4559-256 and 4559-257. The samples were collected during a follow-up inspection at the site on 4-26-18. The Department had listed in the remarks section the reported results of Aluminum at 148.10 MG/L and Lithium at 16.74 MG/L for sample #4559-255. The actual reported results for Aluminum as indicated in the PADEP Bureau of Laboratories Analytical Report is 14.810 MG/L, and Lithium is 1.674 MG/L, for sample #4559-255. For sample #4559-
2749226 Primary Facility 2018-07-05 Routine/Complete Inspection No Violations Noted Weather was 78° and cloudy at time of inspection. Chesapeake representative Kyle Oetting was on location at time of inspection. Well had digital pressure monitoring equipment on the production casing annulus, the production casing and the production tubing. The gauge on the tubing read 985 psi, the casing gauge read 1349 psi and the production casing annulus gauge read 1 psi. There was 1 other annulus vent plumbed to surface at time of inspection. It had 0-1500 psi pressure gauge attached that read 0 psi at time of inspection. The mouse hole had a steel plate welded over it at time of inspection. Performed a walk around inspection of location. No violations were noted at time of inspection.

2820863 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Four wells are staged for production with four GPUs and sand separators. Two steel produced water tanks are present and are moderately corroded. They sit within steel containment with plastic lining. The containment appears to be in good condition overall. The corner of the pad containing the produced water tanks is lower than the rest of the pad and is collecting a large volume of water which surrounds the containment of the produced water tanks. The conductivity of the water both inside and outside of the containment was less than 200 uS/cm.

Two infiltration berms are present and appear to be functioning as designed. Areas within the LOD are well-vegetated and stable.

No E&S issues noted. No violations noted.
