2308436 Primary Facility 2014-09-30 Routine/Complete Inspection No Violations Noted
2359163 Primary Facility 2015-04-02 Routine/Complete Inspection No Violations Noted There was no activity at the site during the inspection. Secondary containment is still in place along with frac tanks which stored fresh water and brine during hydraulic fracturing operations.
2372607 Primary Facility 2015-05-20 Routine/Complete Inspection No Violations Noted Arrived @12:50, weather cold, cloud, windy. This is a 6 slot set-up with 6 wells drilled. None of the wells appeared to be in production. For each well: 13-3/8” x 9-5/8” venting to surface no combustible gas. 9-5/8” x 5-1/2” plumbed into production tanks. Left @13:20

2686288 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted Two wells in production phase. Four wells placed in regulatory In-Active status. Two each sand and water separators. No leaks detected from production equipment. No violations noted.
2702228 Primary Facility 2018-03-05 Routine/Complete Inspection No Violations Noted Arrived @13:20. weather: cloudy, light snow on the ground. This is a 6 slot set-up with 6 wells drilled. 2 wells in production and 4 TD'ed wells with Regulatory Inactive Status. There is liner in place in the well head area. No violations were observed at the time of this inspection.
2768607 Primary Facility 2018-08-29 Drilling/Alteration No Violations Noted The Jes N Bra 1H, N Bra 2H, N Bra 3H and S Bra 1H are under going Frac Ops. The zipper frac operation is starting stage 6 and it's estimated to be completed in the next few days. A above ground fresh water storage pond is in use on the location. Containment around equipment is satisfactory. No violations noted.
2829614 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted Chesapeake Appalachia submitted a Well Site Restoration Report (WSRR) for the JES pad. The WSRR was received by the Department on 1/14/19. Arrived onsite for at 12:21; the weather was ~20°F and calm. The gate was open when I arrived.

There were 6 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. There were 6 GPUs and sand separators onsite. There were also 2 produced water tanks onsite and in secondary containment.

Multiple areas on the inside of the containment berm lacked sufficient vegetative cover. There was also a slip that had formed on the southern edge of the pad. Parts of this slip were sparsely vegetated and it was unclear if this was due to newly formed erosional features or if the winter conditions were preventing proper vegetative growth.

The Department recommends returning the WSRR and seeding and monitoring the berm stabilization as needed. The WSRR could be resubmitted in the spring or summer once the site is completel
