2387074 Primary Facility 2015-07-07 Drilling/Alteration No Violations Noted Arrived at 11:00 with OGI Andy Becher. Weather: humid and hot after rain earlier. Met with Company Man Terry Hooper. For the S Bra 1H well, (015-23017) Nomac 75was setting production casing. At the time of our inspection, 13,464’ of casing was in place with a further 800’ remaining. A casing rotation tool was being used and fluid was being circulated to insert the casing. BOPs were tested on 7/1/15.
2391496 Primary Facility 2015-07-02 Drilling/Alteration No Violations Noted Nomac Drilling Rig #75 is currently in the process of drilling the vertical section of the Folta S Bra 1H at a depth of (3,609) feet. In the backyard area, I observed residue on the containment covering the site perimeter berm and vegetation on the top of the berm at the top of the site cut slope. The residue is grayish in color and appears to be drill cuttings mixed with lime as part of the Pug mill process, either overtopping the containers storing the cuttings or the cuttings being transported out of the containers by wind. Lime is utilized as a drying agent for the cuttings. Fluid levels in (2) containers on containment in the backyard appear to exceed the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56. The containers contain storm water accumulated on containment and drilling wastewater according to Mr. Hooper.
2397663 Primary Facility 2015-08-03 Follow-up Inspection Pending Chesapeake reported bubbles in cellars of S Bra 1H, S Bra 3H. Confirmed bubbling, unable to ID gas.
2398515 Primary Facility 2015-08-05 Follow-up Inspection Violation(s) Noted Bubbling outside of surface casing
2418219 Primary Facility 2015-10-19 Follow-up Inspection Outstanding Violations - No Viols Req'd Water in cellar, no bubbling observed
2448278 Primary Facility 2016-02-01 Routine/Complete Inspection No Violations Noted
2477341 Primary Facility 2016-04-19 Follow-up Inspection Outstanding Violations - No Viols Req'd 20” x 13-3/8”: venting through 2” check valve – 90% methane

13-3/8” x 9-5/8”: venting through 2” check valve – 100% methane. 9-5/8” x 5-1/2”: shut-in

2529253 Primary Facility 2016-10-05 Routine/Complete Inspection No Violations Noted I conducted an inspection of the site accompanied by Andrew Hobbins, DEP O&G WQS of NCRO in Williamsport. Chesapeake submitted a stimulation notice to the Department on 9/16/16. The notice indicated that CHK would be moving to the Folta Pad between 6:00 AM and 12:00 PM from 9/19/16 to 9/20/16. The Folta S Bra 1H, Folta S Bra 3H, Folta N Bra 3H and Hattie N Bra 3H wells will be stimulated. A coil tubing unit is on the location. We met with Ashley Adcock and Tim Long, completions consultants of Chesapeake. Mr. Adcock and Mr. Long stated the following. "The coil tubing unit is in the process of cleaning out sand in the Folta S Bra 1H well. The well has had some issues after the frac was completed. Frac operations will resume when the sand cleanout of the well is completed. The secondary containment is Jersey Barriers, Rig Mats with A (30) mil liner below the mats and a layer of felt below the liner. There are no wells in production. They are shut in. The Folta N Bra 2H and S Bra 2H were in production
2545727 Primary Facility 2016-12-13 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived @ 11:27. Weather: sunny and windy. This site had a total of 6 wells spudded: 4 wells TD’ed in July 2015 and two producing wells. On July 31, Chesapeake reported bubbling in cellars of the Folta S Bra 1H and Folta S Bra 3H, 2 of the 4 recently drilled wells. Previous inspections confirmed this bubbling (08/03/15), documented sample collection by Chesapeake (8/5/15) and documented the results of a 72-hour PBT (10/19/15). For this inspection, accessible annular vents were monitored using an Altair 5X gas meter calibrated on 1/27/16.

Folta S Bra 1H:

20” x 13-3/8”: venting through 2” check valve – 84% methane 13-3/8” x 9-5/8”: venting through 2” check valve – 100% methane. 9-5/8” x 5-1/2”: shut-in

2547237 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted
2637660 Primary Facility 2017-09-18 Routine/Complete Inspection No Violations Noted On September 18, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Folta Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was stabilized with a combination of rock and vegetation. No erosion issues were observed. PCSM has not been addressed at the site.

Six wells were in production during the inspection and all six were labeled. Six GPUs and two production tanks were also on site. Puddles on the well pad surface were low in specific conductivity. No staining was observed.

No violations were observed during the inspection.

2670117 Primary Facility 2017-12-05 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 12:00. Weather: rain. 6 wells in production. Well heads and flow lines recently painted white. Not possible to check for annular gas.

South 2H: top gauge: 1541.8 psi (digital), North 2H: top gauge: 1230.4 North 3H, top gauge: 1380 psi, minor bubbling at threads of 2" bull plug on surface casing dump line. Verifiedmethane with Altair 5X gas meter and called to inform Eric Hottenstein. Hattie N Bra 3H: 2390 top gauge, Folta S Bra 1H: couldn't read top gauge.,Folta S Bra 3H: couldnt read.

2787904 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection. The areas around the well pad were well vegetated or rock lined. No E&S issues were observed.

There are 6 properly tagged Marcellus wells staged for production. There are 6 each GPUs and sand separators onsite and in secondary containment. There is 1 double walled fiberglass produced water tank onsite. It may be advisable to maintain a more traditional secondary containment to avoid issues while emptying produced water tanks.

PCSM/SR plans depict 2 compost filter sock (CFS) sediment traps on the east side of the pad (Photos #2, 3). At the time on inspection some water was being retained in these areas but no CFS was evident. An email was sent to Eric Haskins to check the status of PCSM.

An email received from Eric on 10/11/18 stated that PCSM has not yet been constructed. Chesapeake is currently addressing uncompleted PCSM on several sites.

No violations noted.

2816836 Primary Facility 2018-12-17 Routine/Complete Inspection Outstanding Violations - Viols Req'd Wells shut in for pressure build up test (PBT).

Cool windy at time of inspection.

2818009 Primary Facility 2018-12-20 Routine/Complete Inspection Outstanding Violations - Viols Req'd Wells shut in for pressure build up test (PBT).

Cool windy at time of inspection.

On site with leidos and chesapeake to collect gas samples from annulus vents, and record pressures on annulus vents after 72 HR pressure build up test.
