There were 6 wells on location,6 production units, and a total of 7 cellars. Two wells were in production: Sivers 5H (015-20619) and Sivers 1H (015-20637). The other 4 wells were shut-in. At the time of my inspection, the well enclosure gates were open. Staff with RLE Enterprises were onsite working on a PCSW infiltration system off the pad. Left @ 13:00. 2586771 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted On April 24, 2017, at 1250 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC Sivers Location in Tuscarora Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. An operations worker was on site equalizing the production tanks during the inspection.

The site sign, E&S plan and weekly inspection logs were on site and available for review. The slopes of the pad were observed to be well vegetated. No erosion issues were observed. Four PCSM infiltration areas were observed to be installed at the Sivers Location.

Two wells were in production phase during the inspection. Four more wells have been drilled at the site, but were granted inactive status on 8/26/2015. The two producing wells were labeled and no bubbling was observed in the well cellar. The inactive wells did not have labeling with the well permit number. The Department recommends labeling the wells. Two production tanks and six GPUs were observed at the site. T

2699023 Primary Facility 2018-02-26 Routine/Complete Inspection No Violations Noted On February 26, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Sivers Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last inspection performed by Chesapeake occurred on 2/22/2018. The site was mostly well vegetated. Logging, unrelated to Chesapeake's operations, near the LOD has caused ruts and loss of vegetative cover. The well pad has not been reduced in size, but PCSM has been addressed at the site. Three infiltration areas have been installed around the pad and one has been installed along the access road.

Two wells were in production during the inspection. Four more wells were drilled, but not connected to production and have been approved for inactive status. An orange/brown film was observed in the cellars of several wells. There was also orange/brown staining observed on the

2734491 Primary Facility 2018-05-08 Routine/Complete Inspection Pending Casey Baldwin, DEP O&G WQS and I arrived at the site at 12:24. The weather conditions are partly cloudy skies, warm with a slight breeze and temperatures in the 70's. Hydraulic fracturing (stimulation) operations are in progress. BJ Services, LLC is the CHK subcontractor in the process of conducting frac operations. We met with Deismond Bufford, completions consultant of CHK in the company man trailer. Mr. Bufford stated the following in reference to site activities. Zipper frac operations are in progress. The wireline is on the 3H well. Frac is on the 2H. The frac started here on the 4/22/18. There are (4) wells to frac. We are on stage (22) of (32) stages on the 2H and stage (23) of stage (33) on the 3H." The Department recorded elevated conductivity readings in water on the pad surface next to the containment for an orange in color frac tank. This tank is staged next to the site perimeter berm on the cut side of the pad. It appears as if is water has flowed under the containment for the tank and
2741867 Primary Facility 2018-06-15 Administrative/File Review Violation(s) Noted This administration inspection report is created to document the Department's receipt of sample results related to the discovery of elevated specific conductance (conductivity) readings recorded in water on and off secondary containment during the Department's 5-8-18 inspection conducted at the site. The Department collected a sample on the pad surface as a result of the elevated conductivity readings. For additional details regarding the conductivity readings and samples collected refer to the Department's Sivers 5-8-18 inspection report inspection ID # 2731563. The Department returned to the site on 5-16-18 to conduct a follow-up inspection. CHK had informed the Department during the 5-16-18 inspection that the pad surface was remediated in the general area where the Department had recorded elevated conductivity readings and collected the sample on 5-8-18 inspection report. Fluid had leaked out of blenders used during (hydraulic fracturing) stimulation of wells onto secondary containment, eventually to the
2741914 Primary Facility 2018-05-16 Follow-up Inspection Pending I arrived at the site at 12:32 PM. The weather conditions are cloudy skies, light winds and temperatures in the 60's. Rainfall has recently ended. I met with Nick Krut and Danielle Schaad of CHK. Hydraulic fracturing (stimuation) operations have been recently completed. CHK is in the process of breaking down frac equipment and setting up production equipment. The tanks remaining on location are labeled brine. They actually contain a mixture of flowback and production fluid from various CHK wells. An additional tank contains this mixture and chlorine dioxide from the Mavrex unit. A section of the containment designated for frac equipment, in particular the blender and the frac tank storing fluid treated with chlorine dioxide to be used during stimulation of the wells, has been removed. The rig mats and main section of the secondary containment designated for frac operations is still in place. 2745893 Primary Facility 2018-06-26 Administrative/File Review Outstanding Violations - Viols Req'd This administration inspection report is created to document CHK's discovery of discrepancies in the Department's 6-15-18 inspection report for the Sivers 2H Bra well, inspection ID # 2741867. Eric Hottenstein, Environmental Supervisor of CHK, forwarded an email to me on 6/25/18 at 13:33 requesting a revised version of the inspection report. The email contained the 6-15-18 inspection report with the discrepancies highlighted in yellow in the remarks section of the inspection report. The Department had listed sample analysis results in the remarks section for sample # 4559-258. The samples were collected during a follow-up inspection at the site on 4-26-18. The Department had listed in the remarks section the reported results of Aluminum at 44.82 MG/L, Iron at 95.48 MG/L, Lithium at 16.74 MG/L and Sodium at 11.70 MG/L for sample #4559-258. The actual reported results for Aluminum as indicated in the PADEP Bureau of Laboratories Analytical Report is 4.482 MG/L, Iron is 9.548 MG/L, Lithium is 5.414 MG/L
