Surface and Intermediate casing jobs went as expected with cement returning to surface on both. Location clean and organized. Rig drilling one more hole on location and then moving onto another pad. No Violations Noted at this time.
2406154 Primary Facility 2015-08-25 Routine/Complete Inspection No Violations Noted Nomac Drilling Rig #75 is currently drilling the Johnson Bra 4H well at (6,762) feet. The Johnson Bra 2H and 5H wells were in production, but are currently shut in. The Johnson Bra 3H has been drilled to total depth relatively recently. Water accumulated in the pad surface next to the containment for the flare stack was (2.5) milliSiemens/cm³ in conductivity and (4) milliSiemens/cm³ in conductivity on containment in the backyard near the containment berm next to the site perimeter berm. The water is discolored and appears to contain drill cuttings mixed with lime as part of the Pug mill process, either overtopping the containers storing the cuttings or the cuttings being transported out of the containers by wind and mixing with rain water. Lime is utilized as a drying agent for the cuttings. It as appears as if calcium chloride powder has been spilled to containment and close to edge of containment and the ground surface in the chemical storage area.
2406861 Primary Facility 2015-09-11 Routine/Complete Inspection No Violations Noted Arrived @ 14:00. Weather was sunny and warm following rain the previous day. There were 4 wells spudded on the pad. The 2H and 5H, producing wells, were shut-in. The 3H was recently drilled. Nomac 75 was working on the 4H. I met with company man Darryl Smith. At the time of my inspection NOMAC 75 was conducting an open-hole sidetrack in the lateral. Prior to the inspection the drill string had gotten stuck in the lateral at approximately 17,297’ (TD: 18,854’). The drill string had been severed at 12,200’ and the open- hole sidetrack was at 10,545’.

2706966 Primary Facility 2018-03-16 Incident- Response to Accident or Event Violation(s) Noted On March 16, 2018, the Deparrment was notified about a 100 gallon diesel spill at the Johnson Location by Eric Hottenstein of Chesapeake Appalachia LLC at approximately 1230 hrs. The Johnson well pad is located in Monroe Township Bradford County. The Department conducted an inspection in response to the spill notification. Nick Krut was on site from Chesapeake. Contractors and representatives from Leidos were on site working to remediate the spill when the Department arrived.

According to Mr. Hottenstein, a tractor trailer hauling a frac pump went into a ditch. After being pulled out of the ditch and making its way to the Johnson Pad, one of the saddle tanks that was filled with diesel began to leak. Approximately 100 gallons was lost from the tank. Remediation of the contaminated soil was started shortly after the spill was reported. Contaminated soil was excavated and placed into poly lined dump trucks. The dump trucks filled with contaminated soil were being staged at the Dan Ellis Pad located

2707165 Primary Facility 2018-03-19 Follow-up Inspection Outstanding Violations - Viols Req'd The access road was dug to a depth of ten feet to remove all of the 100 gallons of spilled diesel and contaminated material. The material is being stored at the Dan Ellis well pad until it can be analyzed for landfill disposal. Access road backfilled to original contour. No spilled diesel noticed during inspection. Violations continuing for Environmental Clean-ups program. No samples collected.
2715970 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:24. The weather conditions are sunny skies, with a slight breeze and temperatures in the mid to upper 40's. CHK submitted a Stimulation Notification per 58 Pa. C.S. 3211(f)(2) to the Department on 3/15/18. The notification indicated that CHK would commence stimulation operations on the Johnson Bra 3H well between 5 PM on 3-16-18 and 8 PM on 3-7-18. Shortly after I arrived, I met with Deismond Bufford, site company man, completions consultant of CHK in the company man trailer. BJ Energy Services, LLC is the CHK subcontractor at the site in the process of conducting hydraulic fracturing (stimulation) opearations. Mr. Bufford provided me with the following details regarding site activities. "The Johnson Bra 3H is being stimulated. The Johnson 2H and 5H were in in production, but are currently shut in. We started here last week or weekend. We are on stage (32) currently and (55) stages will be the max with (6) frac stages a day for this well. There are (2) AST's storing water used for
